Subject: Canadian Visitor Visa Application

I, the undersigned, H. G. Pradeepa Manjula Kariyawasam is a Sri Lankan Citizen applying for a Canadian
Visitor Visa to spend time together with my friend Mr. Menan Sivagnanam who is a Canadian Citizen who’s
residing at No. 84, Oakhaven Drive, Scarborough, Ontario, M1V 1X9, Canada, invited me to stay at his residence.
He’s professionally associated at Mike Building Maintenance Ltd., Scarborough, Ontario.

The main purpose of my visit is to reunion with my friend after 15 long years. Accordingly, I have planned to visit
Canada for two weeks on August 2024, that’s from 15th to 28th and assure that I have no intention to stay in
Canada beyond stipulated dates because I have strong ties to my home country and plan to return after my visit.

Professionally, I have served the Sri Lanka Government for the last 34 years and now a pensioner after my retirement
in December 2023. Presently, I’m employed at Maruthi Holidays (Pvt) Ltd., Dehiwala as the Tourism
Development Promoter from January 2024. I have also informed my employer about this tour and have obtained
necessary leave.

Also, I have attached supporting documents for the confirmation of bank balance maintained in Commercial Bank
of Ceylon PLC, Rajagiriya Branch, that is USD 4,100 in savings account & USD 16,900 in fixed deposit
respectively. In addition, I have another residence at No. 10A, Uswatta Mawatha, Ethul Kotte situated
in Colombo District and have attached its valuation report with my visa application for your kind consideration.

Back at home I have commitments with my family since, my wife Piyanka Chandimali Subasinghe is a
government school teacher serving one of the leading girls’ schools in the island and my only son H. G. Jinal
Chamudya Kariyawasam (19 years old) is presently following a full-time diploma course in Computer Network
Engineering at National Institute of Business Management (NIBM), Colombo 07.

Based upon above, I hope that you will consider my application favorably and grant me the temporary visitor visa
before intended travel dates and certainly my visit to Canada will bring me splendid experiences. Please contact me
