228.186(k)(1-4) (k) Controlling pests. The presence of insects, rodents, and other pests shall be control to
eliminate their presence on the premises by:
(1) routinely inspecting incoming shipments of food and supplies;
(2) routinely inspecting the premises for evidence of pests;
(3) using methods, if pests are found, such as trapping devices or other means of pest control as
specified under §228.204(b) and§228.208(b) and (c) of this title; and
(4) eliminating harborage conditions.

OUT 39. Utensils, equipment, & linens; properly used, stored, dried, & handled

Observed cutting boards need to be replaced.

§228.111. Equipment, Maintenance and Operation. Cutting surfaces. Surfaces such as cutting blocks and
boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively
cleaned and sanitized, or discarded if they are not capable of being resurfaced.

§228.113. Cleaning of Equipment and Utensils. Equipment, food-contact surfaces, nonfood-contact


surfaces, and utensils. (1) Equipment food-contact surfaces and utensils shall be clean to sight and
touch. (2) The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted
grease deposits and other soil accumulations. (3) Nonfood-contact surfaces of equipment shall be kept
free of an accumulation of dust, dirt, food residue, and other debris.

228.186. Premises, Buildings, Systems, Rooms, Fixtures, Equipment, Devices, and Materials. (a)
Repairing. The physical facilities shall be maintained in good repair. (b) Cleaning, frequency and
restrictions. (1) The physical facilities shall be cleaned as often as necessary to keep them clean. (2)
Except for cleaning that is necessary due to a spill or other accident cleaning shall be done during
periods when the least amount of food is exposed such as after closing.

(f) Drying mops. After use, mops shall be placed in a position that allows them to air-dry without soiling
walls, equipment, or supplies.

R 46. Toilet Facilities; properly constructed, supplied, and clean

Observed trash lid missing in women bathroom. Toilet room receptacle, covered. A toilet room used by
females shall be provided with a covered receptacle for sanitary napkins.

§10.02.008 Conditions of Retention , Responsibilities of the Permit Holder. Upon acceptance of the permit
issued by the regulatory authority, the permit holder in order to retain the permit shall: (1) post the
following items in a location in the food establishment that is conspicuous to consumers and in public
view:
1. The most recent inspection report (front door)
2. The Certified Food Manager certificate
3. Current permit
4. The most recent inspection scorecard (front door)

Failure to post will result in a written citation

§10.02.008 Conditions of Retention, Responsibilities of the Permit Holder. Upon acceptance of the permit
issued by the regulatory authority, the permit holder in order to retain the permit shall: (10) be subject to
the administrative, civil, injunctive, and criminal remedies authorized in law for failure to comply with
these rules or a directive of the regulatory authority, including time frames for corrective actions specified
in inspection reports, notices, orders, warnings, and other directives
