Applied Activity 4 – Primary Methods

Important
This activity will assess you on the material from learning outcome 3 "use primary research methods to
collect data”. Specifically, the assessment will focus on the objective:

3.5 Describe, select, and use appropriate questionnaire methods.

This assessment should take between 3 – 5 hours in total to complete. There are four submissions for
this assessment (see the schedule in the Overview section). Late assessments will not be accepted and
will result in a grade of 0.

You may not work with another student and/or collaborate on your work or use any material created
by another person without acknowledging the source (plagiarism). It is important that you DO NOT use
AI (artificial intelligence) tools to complete this assessment.

Overview
In this applied activity, you will create a questionnaire to collect primary data, be a participant on two
questionnaires, and provide feedback to your peers to help them improve their questionnaire. Today,
electronic questionnaires are used extensively in research, so it is important to become familiar with
these tools. Please note that your target group should ONLY be students at CAMBRIAN COLLEGE.
Follow these instructions carefully to create an online questionnaire by following these instructions
carefully.

Here are the steps to complete Applied Activity 4. Pay close attention to where (resource) and when
(Due Date) to make a submission at specific steps.

Instructions
Review and follow these instructions carefully to complete the required tasks for Applied Activity #4.
Here are the tools you will use to complete the tasks and make submissions: Microsoft Forms, Moodle
Workshop activity, Moodle Dropbox, Moodle quiz (for reflection).

1. Prepare

 Gather the information for your research topic (research objective and question) that you used
for Applied Activity 1 and 3. You may NOT change your research topic and/question for this
applied activity.

 You will use Microsoft Forms to create your questionnaire. Make sure you can access
Microsoft Forms by visiting forms.office.com and logging in using your Microsoft 365 school
credentials through Cambrian.

 The Moodle Workshop feature will be used to facilitate the submission of your DRAFT
questionnaire (step 4) and the feedback you will submit after completing/reviewing the
questionnaires of two other students (at random). Here are two videos demonstrating how to
use the workshop feature in Moodle. Please review these videos prior to attempting your
submission in the Workshop.

o IMPORTANT NOTE: It is important that you do not miss the due date for submitting your
draft (step 4) and providing peer feedback (step 5) as the Moodle Workshop automates
the collection and distribution of submissions for other students to review and provide
feedback.

2. Set up the Microsoft Form

 You must use ONLY Microsoft Forms to create your questionnaire.

 Visit forms.office.com and login using your Microsoft 365 school credentials through Cambrian.

 Create a “New Form”. For help with Microsoft Forms, please watch THIS VIDEO or visit the
Microsoft Help page.

 Add a title to your questionnaire by replacing the “Untitled form” text at the top of the form.

3. Develop a Draft Questionnaire

Follow these instructions to develop EACH section on your questionnaire (using Microsoft Forms):

 Introduction - Write an introduction in the “Form description” section (that appears after you
click on the title) to obtain consent. Please use the “Consent Form” template found below on
page 9 and fill in the blanks with your research project information. Please note that your
target group should ONLY be students at CAMBRIAN COLLEGE.

 Consent - Create a new section in your Microsoft Form called “Consent” with a multiple choice
question that asks if the participant read the informed consent and agrees to participate.

 Demographic Information - Create a new section in your Microsoft Form called “Demographic
Information” and create at least TWO (2) demographic questions. These could be related to:

o Age
o Income
o Gender
o Highest Education Level

 Topic Questions - Create a new section or sections in your Microsoft Form that are
appropriately titled with questions specifically related to your research topic. You must create
at least eight (8) questions in this section(s).

o These questions must have an appropriate type:


 Multiple choice
 Rating
 Likert
 Ranking
 Text (only when necessary, otherwise always use close-ended formats!)
 Do NOT use open-ended, double negatives, double-barreled, ambiguous,
leading, loaded, recall dependent, and socially desirable questions.

o When you think you are finished your questions EDIT them again and again to make
them as clear as possible and free from bias.

o Review the sequence of the questions so that the funneling technique is followed (from
easy to difficult and from general to specific). You may need to change the order of
some questions. You may decide to also reorder the sections so that the demographic
questions are at the end or beginning of the questionnaire depending on what types of
questions you asked.

 Conclusion - Create a new section in your Microsoft Form called “Conclusion”. Write an
appropriate conclusion to your questionnaire that reminds the respondent to check that they
have not missed any questions and thanks them for their time.

 Note on Formatting: Make sure the questionnaire is neatly aligned and visually appealing (the
overall flow of the questionnaire is clear). The background and colour can be edited with
“Style” options. Please make sure that the full informed consent is still visible if you change the
style of the Microsoft Form.

4. Submit Your Draft Questionnaire – DUE November 09,


2024

 You will now submit a link to your questionnaire in the Moodle Workshop.

 To get the link to your questionnaire, press the “Collect Responses” button, make sure
“Anyone can respond” is selected, press the “Copy Link” button. Please consult the screenshot
and/or THIS VIDEO for how to retrieve the shareable link on Microsoft Forms.

 Once the due date has passed, the Moodle Workshop will randomly assign your questionnaire
to two students who will complete it as a participant and review it to provide Peer Feedback.

You will also be assigned two questionnaires to complete and review (see step 5).

 Instructions on how to make a submission in the Moodle Workshop are provided in Step 1 –
Prepare.

5. Peer Feedback – Complete and Review – DUE November


12, 2024

 Return to the Moodle Workshop.

 You have been assigned two questionnaires submitted by other students. In the Moodle
Workshop, you will see the link to the Microsoft Form questionnaire AND Feedback
(Assessment) Questions.

 For EACH Questionnaire: (There are 2 requirements)

1. Follow the link to the questionnaire and complete it as a participant of the research
study. Take a screenshot of the final screen (you will need to upload this to one of the
Feedback Questions).

2. In the Moodle Workshop, fill out the Feedback Questions for the questionnaire you just
completed. Be sure to upload the screenshot of the final screen in the questionnaire. If
you forgot to take the screenshot, complete the questionnaire again.

6. Revise Your Questionnaire

 Once the due date for Peer Feedback has passed, the Moodle Workshop will close, and you
will be able to see the feedback you’ve received from other students and the instructor.

 Review the feedback carefully and make any necessary adjustments to your Microsoft Forms
questionnaire.

7. Submit the Final Questionnaire – DUE November 16,
2024

 Now that you have revised your questionnaire based on the feedback you received, submit the
Microsoft Forms link in the Moodle Dropbox.

8. Reflection – DUE November 16, 2024

 Refer to your own Questionnaire and choose FOUR of the topic questions.

 Complete the Applied Activity #4 – Reflection quiz in Moodle.
