Conduct the toolbox talk at well site

The job supervisor will conduct a “toolbox talk” with


all personnel involved to discuss among others:
 Job specific procedure and Job Safety Analysis (JSA)
 Installation, validation and operation of barriers
 Potential modes of failures and remedial actions
 Well control procedures (if this applies)
 Line of command for reporting and escalating well
integrity incidents
Proceed with the job ONLY if the risk has been
reduced to ALARP and it is safe to do so

Define and explain the “line of command”

Line of command: the agreed path of communication established and


known by all to escalate and manage well problems & emergencies

When there is a lack of barriers (less than 2), stop the job,
evaluate the risk (HARC), and get permission to proceed.
