SUMMATIVE ASSESSMENTS IN ALL GRADE LEVELS

This Office disseminates the enclosed MEMORANDUM DM-OURO-21-0304,


dated July 30, 2021, REQUEST FOR DATA ON THE RESULTS OF THE
QUARTERLY SUMMATIVE ASSESSMENTS IN ALL GRADE LEVELS, issued by
ATTY. REVSEE A. ESCOBEDO, Undersecretary Field Operations, Palarong
Pambansa Secretariat, and DEACO and MALCOLM S. GARMA, Director IV, OIC,
Assistant Secretary, National Academy of Sports and Field Operation.

In this connection, this Office respectfully requests all Schools Division


Offices (SDOs) to accomplish and submit the attached template on or before August
5, 2021 (Thursday) via email at [email protected]. Submissions
should be CONSOLIDATED at the division level, verified, and signed by the Schools
Division Superintendent.
Please be guided with the attached template and Memorandum, and for
