    How to earn a reward


    We make it easy for you:
    1 Complete the activity.
    2 We receive verification that the activity was completed.
    3 We send you a gift card.

    2024/2025 Hoosier Care Connect Rewards


    Program Who can earn it
    New to the health plan? Take your health needs screening All new members,
    during your first 90 days and get a $35 gift card. Call us to all ages
    take the survey.

    Visit the dentist twice a year. One $25 gift card earned for All members, all
    your first check-up. ages

    See your primary medical provider (PMP) for a well visit or All members, all
    check-up. One $25 gift card per year. ages

    Stop using tobacco products*. Complete a program like All members, all
    1-800-QUIT-NOW (1-800-784-8669). ages
    *You must call in to get this $25 gift card.

    Get your child immunized! One dose of meningococcal, Members who turn
    Tdap, and complete human papillomavirus vaccines by their 13 years old in

    Attend follow-ups after an overnight hospital stay due to Members age 6
