AND IN THE MATTER OF THE REGISTRATION OF PERSONS ACT. 2015 AND IN THE MATTER OF THE STATUTORY DECLARATION TO CONFIRM THE DETAILS OF MY BANK ACCOUNT IN CENTENARY BANK LTD STATUTORY DECLARATION I, ONONO FELIX C/o Sogono Village, Sogono Parish, Kakoro Sub-county, Butebo District do solemnly swear and declare as follows;
1. That I am a male adult Ugandan of sound mind resident of Sogono
Village, Sogono Parish, Kakoro Sub-county, Butebo District the declarant herein and make this declaration in that capacity.
2. That while enrolling for mass National ID I was registered by the name ONONO FELIX having been given the said name by my parents at birth and the same was issued in the same names. (See copy of the National ID card “A”).
3. That when I was opening an Account with Centenary Bank Ltd, I
opened in my very names of ONONO FELIX.
4. That I am fully aware of the source of the Donor’s source of income,
the same not involving any criminal enterprise of any kind.
5. That I also acknowledge full responsibility I bear as the CEO of
PIZZA & CHICKEN CURTAIL.
6. That I have no criminal record in regards to money laundering.
7. That in case of any criminal investigations regarding the stated funds on
my account, I will be answerable.
8. That I make this declaration confirming that the funds held on my
Bank Account in Centenary Bank Ltd are not as a result of any criminal sources.
9. That I swear this statutory declaration consciously believing it to be true
in accordance with the statutory Declarations Act cap 22. 10. That whatsoever I have stated herein bellow are true and correct to the best of my knowledge and belief.
