OCCULAR INSPECTION REPORT ON EXISTING INFRASTRUCTURE PROJECTS

PROJECT NAME : REHABILITATION PF VEGGIE BREAD BUILDING

LOCATION : Brgy. Pastrana, Pastrana, Leyte

COMPLETION DATE : March 27, 2019

CONTRACT AMOUNT : P 389,600.85

DATE OF INSPECTION : October 10, 2024

PURPOSE OF INSPECTION : AS PER BARANGAY REQUEST, FOR THE


EQUIPMENT DONATED D.O.S.T. TO BE OPERATIONAL

INSPECTION TEAM : Municipal Engineering Office

RESULT OF INSPECTION

The Municipal Engineering Office conducted ocular inspection on the above-stated


project dated October 10, 2024 and the following observations were noted:

Physical Assessment Remarks


Building Structures
a. Columns, Beams, Walls
 Structural Stability No defects on concrete columns, beams and
hairline crack on interior wall of kitchen area
 Condition of Painting Painting on walls are still in good condition, some
parts of the painted wall show stains, peelings and
minimal discoloration
 Deflections and Deformations All structural members such as beam and posts are
still intact.
b. Doors and Windows
 Functionality of Door Knobs All Door knobs still functional
 Conditions of Doors & Jambs, Fittings Screen doors have been removed; but main panel
door and flush door are in good condition; CR PVC
door is still in good condition
 Condition of Window frames, panels, Windows are in good condition
hinges, locks
c. Roofing
 Condition of Painting The paint coating as per inspection is in fair
condition with some areas that have discoloration.
 Condition of Roofing Panels As per inspection, the barangay council said that
some parts of the roofing need to be sealed due to
water dripping to the ceiling
 Conditions of GI Flashing, Fascia Ridge roll was examined and found that joints are

d. Ceiling
 Condition of Painting Paint coating on the ceiling have visible signs of
wear, discoloration, peeling. Specific areas where
the paint has deteriorated, such as cracks or flakes,
were identified. Presence of stains or water
damage were noted.
 Condition of Ceiling Panels Existing ceiling Panels have cracks, holes and
discoloration.
 Deformations Presence of holes and cracks in the ceiling are
visible.
e. Flooring
 Condition of Concrete Floor Most of the concrete floor is in good condition; one
area of the structure was found with visible
wearing due to thin application of cement for
finished flooring
 Cleanliness Flooring lacks finishing
f. Electrical System
 Availability of Power Supply Structure not yet connected to main power line
 Serviceability of Lights Fluorescent lights are not yet functional; some
lights are detached hanging from ceiling and one
light is missing
 Conditions of Switches and Outlets Switches & Outlets not yet functional
g. Plumbing and Sanitation
 Availability of Potable Water Supply Not yet connected to main waterline
 Serviceability of Lavatory Not yet functional
 Condition of Comfort Room Comfort room is in good condition but not yet
functional
