Monthly Review Meeting #5 was chaired by DP-NHSRCL and
attended by officials of NHSRCL, TCAP and Afcons
Infrastructure Limited. A Presentation was made by Engineer

during MRM covering Project Progress Information, status of
Method Statements, status of Key Staff deployment, and other
salient information.
GENERAL PROGRESS
2. a) Time Lapsed = 9% (168 days) Information -
b) Physical Progress = 5.52 % (Roughly)
PROJECT SAFETY STATISTICS
a) Safety Statistics of the project was presented and reported Safe
Manhours is 3,04,822. Incidents of Near Miss is 1(one) during Information -
the reporting month.
b) The root cause for the observed Near Miss of Dumper body
opening during mucking was informed by contractor due to AFCONS Noted
3. lack of training of driver. Engineer and Employer advised
contractor to commence SHE Third Party training for

c) Employer cautioned the contractor and the Engineer on the
safety incidents and instructed Engineer to implement measures AFCONS -

Project Key Personnel: -
a) Contractor informed that out of total 8 Key persons, 6 have
reported to site. Also, contractor informed that credentials for Information 20.10.2023

b) The Engineer informed that the submitted credential for the TCAP Next meeting

resources at the contractor end. Employer directed the
contractor to submit the design and method statements well in
advance as per rolling program.
Key Equipment’s Deployment Plan
TBM Equipment: -
a) The Contractor presented that the TBM design works are in
final stage at the vendor level. Information -
b) Contractor informed that the revised – preponed date of TBM
03 delivery to site is 11.07.2025 and FAT date to be Information -
23.04.2025. Contractor informed that accordingly updated
CWP is also submitted to the Engineer for review and
approval.
6. c) The dates proposed by contractor shall be reviewed by the AFCONS 10.11.2023
Engineer as per the contractor’s bid and overall construction
schedule.
d) The contractor was instructed to expedite the vendor approval
and design of tunnel segment bolt joints and gasket. AFCONS 10.11.2023
20.12.2023
NATM Equipment: -
a) Contractor informed that the Jumbo Driller for NATM Drilling Information -
at Adit location is mobilized Contractor further informed that
the Excavator and Shotcrete Machine for cut and cover work
are deployed at Shilphata & Adit site respectively .
Work Front wise Progress at SHAFT – 2 16.12.2023
a) Secant piling works 97% (156/160) completed and will be AFCONS 24.12.2023
completed on 24.11.2023
b) Contractor informed that capping beam construction ongoing AFCONS 03.12.23
116m/162 m, 70% is completed, and target completion is 03-12-
2023. Information -
c) Contractor informed that the mechanical excavation along with
side support for waler and anchor in progress. 50% of first
level Weller beam construction completed (84/162 RM). 21.11.2023
d) Engineer highlighted shortage of labour at site leading to delay AFCONS 20.12.2023
7. in Gantry foundation works vis-à-vis the planned rolling
program and advised contractor to increase rebar labour. AFCONS Noted
e) Contractor informed that Soil Anchor works to commence and
2nd Drilling Rig will be mobilized soon.
f) Contractor informed that excavation is in progress for retaining AFCONS 30.12.2023
wall construction for permanent reinstatement works and 60m
length is planned for month of December 2023. AFCONS 28.02.2024
g) Contractor informed that NONOC status for the sub-site office
layout is received from the Engineer and that office
construction will take next months.
8. Work Front wise Progress at SHAFT – 3
a) The Engineer raised concern on the delays in completing the

site hut due to the insufficient resources. Revised target date AFCONS 18.11.2024

b) Engineer raised the concern on the slow progress of
excavation rate at shaft due to insufficient resources
(manpower) at site, frequent equipment breakdown, lack of AFCONS Noted
contractor’s supervisory staff and delay in explosive vans

c) Engineer raised concern on slow progress for overhead gantry

foundation. Also, Engineer highlighted non-submission of
Method Statement for Gantry Erection. Contractor informed 15.12.2023
that the Gantry foundation will be completed by 15.12.2023.
The Engineer further raised concern on the delay of work due
to delay in material and equipment delivery at site. AFCONS Noted
d) Engineer informed that despite NONOC status is accorded to
Contractor’s Muck disposal plan, the shaft muck is not timely
disposed off the site due to contractor’s Royalty issue.

Work Front wise Progress at Casting Yard
a) Contractor informed that 17 Acres of land is acquired and Information -
informed that 30% area levelling/cleaning works completed
and barricading works 5% completed.
b) Contractor informed that pilling works on the batching plant

works completed for water supply at site.
c) Contractor assured that approximately 1 month time was

lapsed in obtaining the Forest clearance for the access road.
And continually assured the mobilization of resources in
required number to expedite the works.
EBS Survey and Instrumentation: -
a) Engineer highlighted that only 2 reports out of total 268
surveyed structures are submitted from contractor end and -
expressed concern on the delay in EBS reporting. Information
b) Contractor highlighted that 121 nos are pending the Engineers
signatures and approximate 147 reports pending with vendor AFCONS Noted
for submission to employer. Employer advised to deploy
additional manpower and expedite the works.
c) Employer instructed, report signing procedure at the
TCAP -
Engineer’s end to be expedited.
12. d) Contractor informed that Instruments for monitoring are
installed/planned to install at all sites progressively before AFCONS 25.10.2023
commencement of excavation works. Contractor further
updated that all instruments at shaft 3 are installed and
instruments for Adit and Main portal are in transit and shall
be installed by 25.10. 2023.
e) Employer directed to prepare a detailed presentation for the 15.11.2023
AFCONS
Instrumentation and monitoring work by next meeting. Next Meeting
f) Engineer advised contractor that Incidents of trigger levels AFCONS

Approval Status of CWP -
a) Contractor informed that Revised CWP was submitted on
18.10.2023 for Engineer review, which was given NOO status Information
by the Engineer.
b) Contractor informed that the major issue observed in the 6.11.2023
submitted CWP is related to the finalization of Docking AFCONS & 30.11.2023
location. Employer advised Engineer and contractor to site TCAP

progress in the program are not justifiable and advised AFCONS
contractor to re-visit the allotted physical percentages.
d) Engineer has requested contractor to present the impact of AFCONS 06.12.2023

06.12.2023.
