Home Healthcare Nursing (HHN)

• Home Healthcare Nursing competencies are integrated knowledge, skills,


judgement and attributes required of a nurse working in home settings to practice
safely and ethically.
• Home health nursing encompasses disease prevention, rehabilitation, restoration
of health, health protection and health promotion with a goal of managing existing
problems and preventing potential ones.

• is a nursing discipline or speciality for the provision of
multidimensional home care to clients/patients of ALL AGES.
• The services are rendered outside the hospital setting, the care is
within the home or home-like environment, it targets reduction in
hospital admission or facility placements while the client recovers/live
with illness, injury, temporary or permanent disability.

•A nursing specialty that involves providing
one-on-one care for patients in their homes.
HOME HEALTHCARE NURSING

Home health and personal care aide are two of the very recent and fastest growing
occupations in the health industry.
HHN is a supportive care that is provided in the home. It allows a person to remain in the
comfort of his/her home while receiving services to recover from illness/or its effects,
injury, or disability. Home care services are also provided for people who have chronic
conditions such as diabetes, HIV/ AIDS, hypertension, heart failure, chronic wounds,
cancers, and chronic obstructive pulmonary disease (COPD), etc. Its also to care for
people who have cognitive or physical disabilities to help them complete activities of
daily living.
Home care may also be provided for patients who have been diagnosed with a terminal
illness.
Indications for HHN
• After hospitalization for surgery or a chronic illness.
• For chronic disease management (e.g., diabetes, heart failure)
• For rehabilitation services (e.g., period of physical therapy)
• For palliative management
• Personal care aides or Home health aides are those involved in
helping patients with basic personal needs such as getting out of
bed, walking, bathing, and dressing.
• Some aides have received specialized training to assist with more
specialized care under the supervision of a nurse HHN.
NURSING TASKS:
• There’s a wide range of inclusiveness in HHN:
• Vitals (T, P, R & B/P, Wt, Ht, BMR), wound dressing, intravenous
therapy, administering medication, monitoring the general health of
the patient, pain control, and other health support.

Responsibilities of Home Health Care Nurse
• Personalized Care Plans: Nurses assess the patient's needs and create
tailored care plans that address medical, emotional, and social
aspects.
• Skilled Nursing Services: Services may include medication
administration, wound care, IV therapy, and monitoring vital signs.
• Patient Education: Nurses educate patients and families about D6
conditions, prognosis, rehabilitation requirements, medications, and
self-care techniques.
• Emotional Support: Providing companionship and emotional support
is crucial for the mental well-being of patients.
• Multidisciplinary Approach to Care: collaborate with physicians,
surgeons, physical therapists, & others to ensure comprehensive care
to meet the needs of individual patients. And care coordination.
Merits of Home Health Care Nursing

• Comfort and Convenience: Patients receive care in a familiar


environment, which can enhance recovery.
• Cost-Effectiveness: HHC can be more affordable than hospital stays or
long-term care facilities.
• Personalized Attention: Patients often receive more one-on-one
attention compared with crowded & larger healthcare settings.
HHN Practice
• One of the great benefits of a career in Nursing brought up by the
recent advancement is the wide variety of employment opportunities
available, including non-traditional work environments like home
healthcare.
• HHN could be an appealing option for independence and autonomy, as
well as a flexible work schedule beyond a typical hospital/Clinical
work.
Reality of HHN

• Home health nursing is a unique field considering the


obstacles faced by healthcare professionals. When out on
visiting clients, the nurse is alone and cannot rely on
others most of the time.
• HHNurse must learn to autonomously make sound
judgements and apt in decision making
CAUTION

• HH services are covered when certain criteria are met.


• HHNs may not perform tasks or duties that are outside of their scope of practice or
competence or that have not been assigned to them.
• There may be situations in which a patient or family requests for something not
incorporated/ trained for or allowed.
• The HHN should intelligently explain that is not allowed or permitted.
