 The gas monitoring to be done initially at the time of issue of the permit
and periodically/continuous if required.
 The exhaust blower should be provided with flame prof equipment.
 The fresh air blower should be provided and should be kept at such
location that it is sucking fresh air and not polluted air /exhaust of the
other equipments.
 The Confined Space Entry Permit not to be issued if the environment
inside is not safe.
 Gas monitoring instrument should have valid calibration.
 Appropriate Respiratory Protection Devices to be used.
 Effective ventilation to be ensured by using air erectors and exhaust
fans.
 Workers should carry portable gas monitor inside the confined space.
 If oxygen concentration is found to be less than 19.5% then provide
safe and suitable arrangements of oxygen supply for individual work
man

Entrapped  Proper entry / exit route to be provided. Supervisor


 The exit/escape route should be kept clear off obstruction.
 The ladder to be lashed with fixed support.
 Full body harness with life line / fall arrestor to be used.
 Entry / Exit register to be maintained.
 Hole watcher / Tally Man / Stand by man should be positioned near
Entry / exit.
 The communication to be maintained between entrant and the stand by
persons.
 Cautionary sign boards to be displayed.
 Don’t wear loose cloths at outside manhole

Exposed to flying grits  Nozzle must be proper, Dead man Switch attached. Supervisor
 Working area shall be barricaded and warning sign posted. Site Engineer
 Un- authorized entry shall be restricted.
 Shot resistant apron, Hand gloves, Blasting coverall & hood connected
with filtered airline.

Release of dust  Adequate exhaust fans shall be provided to through dust outside the Supervisor
blasting shed. Safety Officer
 Grit blasting shall be done in an enclosed space.
 Working area will be located down wind & away from the crowd & other
facilities.
 Used grits shall not be accumulated on ground, immediate
housekeeping shall be done.

In-competent persons  Only trained & experienced persons shall be engaged in blasting Supervisor

 Blasting operator shall be medically fit & be attentive all time.

 Remove all the equipments at the end of day.

Fire & Explosion /  The blasting nozzle will have a dead man switch which operates Supervisor/Blaster
Defective Tools & properly and nozzle is grounded to earth.
Equipments  All hose connections shall be secured with clamps properly with whip
check.
 Air filters shall be replaced when it gets dirty.
Compressor Operation  All electrical fixtures and switches should be in good working condition.
 All electrical tools & equipments shall be inspected by OIL before use
 Affective supervision, Training &Awareness
 Provision of Fire Extinguishers
 Mandatory PPE’s as well as job specific PPE’s Must Wear.

 Double insulated hand tools should be used as well as electrical cable
over hanged to avoid trip Hazard.
 Ensure all tools are inspected by OIL HSE and
Color coded accordingly.
 Waste / Scrap should be collect after completion of day work on
daily basis.

 First Aid box always available at work site.


 Emergency vehicle should be parked near site office.
Medical Facility &  Supervisor should know the emergency contact numbers, assembly
