    AFFIDAVIT OF LEGAL BENEFICIARIES

    I, JETHER P. MISAMEN, single, of legal age, Filipino and residing at


    Bao, Alamada, Cotabato, after having been duly sworn to according to law
    hereby depose and state;

    1. That I am applying in the Military Service as a trainee in the Philippine;

    2. That I was born on January 21, 2002 at Alamada, Cotabato and one of
    the children of SPS. RAMON MISAMEN and ELIZABETH MISAMEN;

    3. That I have never contracted any marriage in the Philippines or


    elsewhere, and I remain SINGLE to this very day and hour;

    4. I have no children whether legitimate or illegitimate;

    5. Accordingly, I am hereby declaring SPS. RAMON MISAMEN and


    ELIZABETH MISAMEN as my legal beneficiaries in all insurance that
    will be due to me and in all pertinent documents that will need the
    declaration of legal beneficiaries; and;

    6. That I am executing this affidavit to attest to the truth of the


    statement above and for whatever legal purpose it may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 8 th day of


    July, 2024 at Manila City, Philippines.
