    Rheumatic Fever:- Treatment of Rheumatic fever:-

    • age 5-15 year 1- for eradication of streptococcal infection


    • preceded by URTI ( pharyngitis ) — 10 days oral penicillin or amoxicillin
    Major criteria :- OR single IM injection of benzathine
    1- Arthritis :- penicillin
    first one appear , big joints , Polyarticular, 2- anti inflammatory Treatment (aspirin &
    Migratory , joint red, hot, swollen, tender, steroids):
    limitation of movement Good response to
    A- RF with mild carditis:
    NSAI, resolved with no deformity
    • give Oral salicylates (aspirin 50-70 mg/
    2-PanCardites
    kg/day)
    3-Rheumatic chorea
    • Total duration & 6-8 wks.
    4- Erythema marginatum
    B- RF with Carditis with cardiomegaly &
    trunk & proximal parts of the limbs.
    CHF:
    large erythematous macules with pale centers
    • give Oral corticosteroids (prednisolone
    And erythematous Borders
    2mg/kg/day)
    5-Subcutaneous nodules
    • total duration & 6-8 wks.
    over the extensor surfaces of limbs
    • give aspirin should be started at 50 mg/
    painless-mobile
    kg/day) when prednisolone is being
    Minor criteria:-
    tapered
    1. Fever.
    3- supportive management
    2. Arthralgia
    • Bed rest
    3. Prolonged P-R interval
    • fluid & salt restriction digoxin in sever
    4- Elevated acute phase reactants
    Diagnosis:-
    cases

    ( 2 major criteria or 1 major + 2 minor 4- prevention of recurrent attacks

    criteria) + Evidence of recent streptococcal Benzathine penicillin IM every 21-28 days


    streptococcal infection Ex:
    1. Positive throat swab.
    2. ASO titer
