Dear Doctor,
Thank you for accepting Ms Patricia Styles, a 62 years old patient who is suspected of a possible
relapse and complication of pericarditis. She requires urgent investigation.
Today, during scheduled home visit, Ms Styles shows no sign of improvement and reports chestpain
relieved by sitting up, shortness of breath and fatigue. She has a low-grade fever, tachycardia 115,
tachypnea and BP of 125/78 which is lower than her usual.
On August 23, 2019, after 3 hours of drive to visit her sister, Ms Styles was admitted to Green Valley
Hospital late in the evening with fever, sharp and pleuritic chest pain which worsen during inhalation.
On the following day, full blood examination shows signs of infection, throat swab reveals type B viral
influenza and echocardiogram confirms pericarditis. She was managed with IV saline and Ibuprofen
600mg every 8 hours. Last August 25, 2019 she was discharged back to her home and advised on self-
care.
Please note that Ms Styles has hypertension, DM Type 2 and depression which are managed with
Quinapril 40mg twice daily, Metformin 500mg twice daily and Gliclazide 30mg daily.
Please see Ms Styles at your earliest convenience for an urgent assessment and management of
possible relapse and complication of her pericarditis.
