1. Introduction about project

PMNDP portal is a comprehensive web based application which supports system to closely
monitor real time data transfer from dialysis facilities to central server of MoHFW. The
Pradhan Mantri National Dialysis Program is part of the National Health Mission (NHM) for
provision of free dialysis services to the poor. The Guidelines for Pradhan Mantri National
Dialysis Program envisages provision of dialysis services under NHM in all the districts of the
country.

2. About User Manual

The User Manual contains all essential information for the user to make full use of the
information system. This manual includes a description of the system functions and
capabilities, contingencies and alternate modes of operation, and step-by-step procedures
for system access and use.

3. User of the applications

The application has three modules at present:

National Module

State Module

Facility Module

The PMNDP Portal can be accessed through https://pmndp.mohfw.gov.in

4. Step wise Process

4.1 Login Page

Step 1; Enter URL as


https://pmndp.mohfw.gov.in/frontend/login or click on the
login button on the Home Page

Step 2; Enter valid username & password


Step 3; Enter Captcha

Step 4: Click on login button: If credentials are correct then login is successful and user can
see the home screen.

Step 5: Forgot password: If user forgets password then enter the recovery email id to
receive new password.

4.2 Dashboard Page

Step 1: Home screen with menu and dashboard: If login successful then new page will open
as below

4.3 Dashboard PMNDP

Click on Dashboard link in the menu navigation from the left hand side to view dashboard
Dialysis services status under PMNDP.

● Hemodialysis Machine: Displays total hemodialysis machines data in the


dashboard.
● Fresh Patient Registration today: Display the total no. of Patient registered
today.
● Cumulative Patient Registration: Displays total no. of patient registered in the
center.
● Cumulative Hemodialysis Sessions Held: Displays cumulative Hemodialysis
Sessions Held in the center.

4.4 Edit/View Health Facility Details

Click on Health Facility Details, you can see List of Facility and edit facility details with these fields
date, state, district, facility location, facility type, facility name, facility address, pin code, In charge
name, In charge mobile, In charge Email Id, NIN Id and Total no. of Functional Hemodialysis Machine
in this Centre and Facility Operation Type.

4.5 Patient Registration

4.5.1 Add New Patient:

4.5.1(a) ABHA ID available: YES

If the Patient already have ABHA id so he/she can verify himself. Click on verify Health ID
a) Choose verification method:
a.1) OTP
b.1) Biometric

a.1) OTP

 Enter ABHA ID/Health ID

 Tick on check box

 Submit
After submit you will be redirect to next page:

 Enter OTP which will be sent to your registered mobile no.


 Click on OK.

Facility will be redirect to next page:

 Click on Save.

b.1) Biometric:-

 Click on Yes
 Choose verification method
 Click on Biometric
 Enter Health ID/ABHA ID and submit
 Tick on checkbox and submit

 Click on click here
 Verify your finger and verification successfully then click on continue after
verification successful redirect to patient profile page.

4.5.1(b) ABHA ID available: NO

If the Patient do not have the ABHA ID. Facility can register the Patient in center with two
methods

 Create ABHA id using Adhaar no.

 Create Patient without using Adhaar no.

With Adhaar no.


a). Choose Aadhaar verification method:
a.1) OTP
b.1) Biometric
c.1) Aadhaar Demographic

 Enter Adhaar no.


 Tick on the checkbox to confirm.
 Submit
 You will redirect to next page: OTP will be sent to register mobile no of patient.
 Enter OTP

 Click on OK.
 ABHA ID will be generated and automatically all related data get populated in the registration
form click on Save Button to register patient.

b.1) Biometric: -

 Click on Yes
 Choose verification method
 Click on Biometric
 Enter Health ID/ABHA ID and submit
 Tick on checkbox and submit

 Click on click here
 Verify your finger and verification successfully then click on continue after
verification successful redirect to patient profile page.

c.1) Aadhaar Demographic: -

 Click on Aadhaar Demographic


 Fill all the details of patient as mentioned in his Aadhaar Card (The details of patient should
also be the same in PMNDP portal too as mentioned in his Aadhaar card).
 Facility will add with Aadhar number, Aadhar based Name, DOB, Gender, and Mobile Number.

 Name, DOB, Gender should match with Aadhar number.
 Aadhar based Mobile Number enter so SMS will receive in Aadhar based Mobile Number.

 Click on Submit.

 Patient will be registered in PMNDP portal with his ABHA identification number.

Create patient without Adhaar no.

 Fill the form and click on save.

The patient details will be visible on patient line listing.

In Patient line listing, you can see List of patient with field’s Patient name, Age, Gender, Mobile no.,
ABHA Id/Unique ID, HD procedures type.

Facility can search, edit, view and disable the Patient from the dashboard

4.5.2 View:
Facility can see patient’s information.

4.5.3 Edit:
Facility can edit any personal information of patient.
Facility will have to add patient’s mobile number at which in case of any emergency can be
contacted.

4.5.4 Linking Abha Id:

If patient is registered in PMNDP without his ABHA ID, Facility can link patient Abha id with their
Adhaar card.
If patient have his ABHA id with him, his ABHA id can be verified, and that patient unique health ID
will be converted into his ABHA id successfully in the PMNDP portal.
If patient have his Aadhaar card with him

A) If Abha ID Available:
a) Choose verification method:
a.1) OTP
b.1) Biometric
a.1) OTP

 Click on Yes
 Choose verification method
 Click on otp
 Enter Health ID/ABHA ID and submit
 Tick on checkbox and submit

After submit you will be redirect to next page:

 Enter OTP which will be sent to your registered mobile no.

 Click on OK.

You will be redirect to next page:

 Click on Save

b.1) Biometric:-

 Click on Yes
 Choose verification method
 Click on Biometric
 Enter Health ID/ABHA ID and submit
 Tick on checkbox and submit

 Verify your finger and verification successfully then click on continue after
verification successful redirect to patient profile page.

b) If ABHA ID not available:

 Click on No

 Enter Adhaar card no and submit.
 Enter OTP which will be sent to your registered mobile no

4.5.5 Deactivate/Activate Patient:

 Facility can deactivate or activate any patient as per the situation or circumstances.
 Click on red button

 Select the reason
 Give remarks and Save.

4.6 Slot Management

In Slot Management, Facility create the slots for available machine. Facility

can edit and view the Machine slots details.

There is facility where facility available slot for public.

4.7 Machine Master:

In Machine master, Facility add the center machines.Facility can edit and view

the Machine.

4.8 Dialysis Module

4.8.1 Hemodialysis

In Dialysis Module, All the registered Patient listing show in the Dialysis module with FreshStatus.

Facility can search the Patient by using Name, Mobile number and Dialysis type.

Facility can schedule the Dialysis of Patient, Start the dialysis and complete the Dialysis.
Facility can also unbook the dialysis of patient if he doesn’t come for dialysis, so that the vacant slot
will be available for needy person.

If patient does not come to take dialysis for any reason, facility will have to unbook the slot on the
same day.

If facility forgets to unbook the slot, unbook option will be after scheduled date, facility will have to
follow process of start and end dialysis but with Failed dialysis status.

At time of START dialysis and COMPLETE dialysis patient will have to enter the patient health record which
includes:
 Blood pressure
 Weight
 Hemoglobin

 Serum Creatinine

4.8.2 Peritoneal Dialysis

In Dialysis Module there are other type of dialysis Peritoneal Dialysis.


Facility will have to click on Peritoneal dialysis, as shown in the screenshot given below.

Facility will be able to search for PD patient through their ABHA ID/ Health ID , patient Name, or Patient
Mobile Number.

Click on Plus Icon, the screen will be visible given below. Facility will have select patient visit date, enter CAPD
Batch No, CAPD Batch Expiry Date, No. of Bags Issued, select last collected date (if patient have taken PD
dialysis earlier), Select next due date.

Facility can upload upload patient prescription too and click on Save.

4.9 Dialysis History:-

4.9.1 Hemodialysis History

In Dialysis History, Facility can see the History of the Patient.

Facility can search the patient with ABHA ID, Dialysis Status, from date, to date and mobile no.

4.9.2 Peritoneal Dialysis History

If facility clicks on PD history, he will be able to see Peritoneal patient dialysis too. Facility user will be
able to search PD patient by his/her Name, ABHA ID/Health ID, From Date and End date or Mobile
Number and click on Search.

Facility will be able to click on Export and download the PD patient report in excel format.

5.0 Waiting List:-

In Waiting Patient listing, you can see List of patient with field’s Patient name, Age, Gender,
Mobile no, Health ID(ABHA ID-Aadhar based),Dialysis Support Type and HD procedure Type.

Facility can search, view and shift the patient from the Dashboard. There are two icons given in
Action: View Patient and icon in green to Shift Patient.

5.1 Add Waiting Patient: -


5.2(a) ABHA ID available: YES

If the Patient already have ABHA id so he/she can verify himself. Click on verify Health ID

5.2(b) ABHA ID available: NO

If the Patient do not have the ABHA ID. Facility can register the Patient in center with two

 Tick on the checkbox to confirm
 Submit
 You will redirect to next page: OTP will be sent to register mobile no of patient.

5.1.2(b)Create patient without Adhaar no.

All the process will be same as patient registration module.

5.1.4 View Waiting Patient: -

Waiting patients will be visible in line listing as given below,

5.1.5 Shift Patient: -

On click on shift patient, the patient of waiting list will be shifted to main patient line listing and as
per availability of slots facility will be able to give dialsis to the patient.

5.3 Reports

1.0 step 1
In Report All States Monthly Progress Report, Facility can see the Details of the States Monthly
Progress Report.

In Click on Reports tab Facility can Select Report Type, from date, to Date.

In select reports type you can see multiple type report (Patient with ABHA ID, Patient
Without ABHA ID, Dialysis Machine List, Patient Hemodialysis Count, Monthly Progres Report).

In Click on Export to excel button Facility can Download All States Monthly Progress Report

5.4- Global Search

In Global Search Patient listing, you can see List of patient with field’s Patient name, Age,
Gender,
Mobile no, Health ID(ABHA ID-Aadhar based), State, District, Facility Name, Waiting Patient.

Facility can search, view, view Dialysis History and transfer the patient from the Dashboard.
There
Are three icons given in Action: View Patient, View Dialysis History and icon in green to Transfer
Patient (Patient data will be transfer own facility).

Facility can search patient ABHA Id/Patient id, Patient Name and Mobile Number any of these.

Facility can see patient’s information.

Facility can see patient’s Dialysis History List.

5.2.0-Transfer Patient:-
Facility can transfer patient data in its own facility.

5.5- Transferred List:-
In Transferred listing, you can see List of Transferred patient with field’s Patient name, Age,
Gender, Mobile no, Health ID (ABHA ID-Aadhar based), Current State, Current District, Current
Facility Name, Transfer Reason.

6.0- HFR module:-
HFR (HEALTH FACILITY REGISTRY) SELF REGISTRATION FORM:

At login there will be a box of REGISTER HFR.

 Click on REGISTER HFR new popup will open.

 After Click on Register HFR one popup will open where facility can enter HFR id.
 Verify HFR
 If HFR exists: OTP will be sent to registered mobile no. through ABDM API

 If HFR does not exists: This HFR does not exist message will be reflect.
 Verify OTP complete form will open
 After validate OTP
 The details will be reflected in the form

 Verify OTP the details will be reflected in the form.


 State, District, Facility Name, Facility Address, Pin Code, in charge Name will display auto fill from
ABDM.
 Click on Register and Submit.
 After complete registration Facility will able to login the PMNDP portal through HRF id.

NIN id attached with HFR you will enter NIN id so some details will open you have two options yes or
no you can see given screen.
