EXCAVATION PERMIT
1. That the applicant shall excavate only after posting a restoration bond in the amount
of One Hundred Fifty Thousand Pesos Only (Php. 150,000.00) in the form of Surety
Bond and Non-refundable amount of Two Thousand Two Hundred Sixteen Pesos &
37/100 Only (Php. 2,216.37) to cover the Engineering and Administrative Overhead
(Supervision Fee) as provided in Department Order No. 26 series of 2011;
2. That, this Office should be notified at the start of the excavation work so that an
authorized representative could be sent to determine the actual area to be excavated.
That, if the applicants exceed the quantity originally applied for excavation, additional
payment will be required corresponding to the area destroyed/excavated;
3. That the applicant shall secure from the utility company/ies the excavation of its/their
utility lines in the site of excavation to avoid destruction of its/their underground
facilities;
4. That, the applicant shall coordinate with the Local Officials concerned of this
excavation/digging in their area of jurisdiction;
5. That, the necessary and standard road signs, barricades and red lights during night
time should be posted within the premises of the work being undertaken to warn the
end-users (traveling public, both vehicular and pedestrians) on the danger posed by
the open excavation, pile of excavated materials & etc. That, the roadway including
the excavated portion must always be conveniently passable at all times;
6. That, the materials and equipment should be properly kept outside of the travel-way
so as not to interfere with the flow of vehicles and pedestrian using the
shoulder/sidewalk;
7. That, the site should be cleared immediately after the completion of the work;
8. That, any consequential damage on the road or any other road structure resulting
from their work should be restored by the applicant to its original state and condition
including damaged thermoplastic pavement marking;

11. That, the work covered by this permit should be completed within Ninety (90)
Calendar Days upon issuance of said permit counted from the date of the Official
Receipt of this office and immediate restoration must be undertaken.
12. That, the applicant shall be liable to any accident that may occur during this
undertaking.
13. That, the applicant shall provide Steel Plate, Concrete Saw during excavation and
restoration works;
16. That, after the completion of the work the applicant shall submit a duly signed AS-
BUILT plan of the project; and
17. That before any payment was made due to the contractor by the MWCI, the same
should secure first a Certificate of Completion of the DPWH;
18. That non-compliance to any of the conditions stated above shall constitute sufficient
ground for the revocation of this permit and that no other permit will be issued to the
applicant, its contractor and MWCI Company in the future with similar nature;
