Acute Coronary Syndrome (ACS) Management Protocol

Step 1: Initial Assessment and Monitoring

Obtain vitals, perform 12-lead ECG, and collect blood samples (troponin, CBC, electrolytes).

Step 2: Immediate Pharmacologic Management

Aspirin: 160-325 mg, chewed.

P2Y12 Inhibitor (e.g., Clopidogrel 300-600 mg, Ticagrelor 180 mg).

Nitroglycerin: 0.4 mg SL every 5 minutes, up to 3 doses.

Morphine: 2-4 mg IV for pain.

Oxygen if saturation < 90%.

Step 3: Anticoagulation

Unfractionated Heparin: 60-70 units/kg IV bolus, then 12-15 units/kg/hr.

Enoxaparin: 1 mg/kg SC every 12 hours.

Step 4: Beta-Blockers (within 24 hours if no contraindications)

Metoprolol: 5 mg IV every 5 minutes (3 doses), then 25-50 mg orally every 6 hours.

Step 5: Statin Therapy

Atorvastatin: 80 mg orally daily.

Step 6: Reperfusion Therapy (for STEMI)

PCI within 90 minutes of first medical contact if possible.

Fibrinolytic Therapy if PCI unavailable: Alteplase or Tenecteplase.

Step 7: ACE Inhibitors / ARBs (within 24 hours if LVEF < 40%)

Lisinopril: Start with 2.5-5 mg orally daily.

Losartan: 25-50 mg orally daily if ACE inhibitors are not tolerated.

Step 8: Additional Measures and Long-Term Management

Lifestyle changes, dual antiplatelet therapy (Aspirin + P2Y12 inhibitor), follow-up.
