FORM 3 (SECOND SEMESTER)

SELF-REFLECTION & EVALUATION

Name: Leher Keswani

Class: 3C

Class No.: 9
Message from your Heads of Year …

Hi Form 3!

This year we have been so proud of you all as you have been growing into young adults. We have seen you
studying hard and taking on the challenge of your new elective subjects and increased academic
expectations. We have seen many of you involved in school activities and take on leadership roles. Lastly, we
have enjoyed seeing you take more responsibility for both your learning and behaviour. We hope that you
can take time during this reflection to celebrate your successes and set yourself some goals for the rest of
the year. If you would like any more help or advice please come and speak with us at any time.

Ms. Angelina Mok and Mr. Anders Lam

Which of the character dispositions above do you think is your strength?

How do you show this disposition in school?

I have put full effort into achieving above a B for all graded things so far this semester.

Other than the one you have chosen above, choose one other character disposition you would
like to develop over the coming months.

Self assurance

How can you develop this disposition in school?

I can develop this by repeating positive affirmations to myself when I am in doubt about my
abilities.
The following learning habits will help you to ensure that you make the most of your lessons and
enable you to stay organised. Take time to reflect on the list below and rate yourself in each area
by putting an “X” in the boxes:

Learning habit Never Sometimes Always

I am punctual to school and lessons X

I bring the correct books and materials to lessons X

I listen carefully in class X

I participate by asking and answering questions X

Suggest three ways to improve your learning habits over the rest of this semester.

2 I can try reading more books related to my books.

3 I can focus more in class.

I can try and achieve all 5.

PART C. Target Setting
After reading your report card and reflecting on your progress so far, it is now time to set some new
targets for Semester 2. Two of your targets should be based on your subjects and attainment, and two
should be based on non-academics such as ECAs, developing character dispositions or improving effort
grades. Make sure that your targets follow the SMART guidelines below.

PART D. Form Goals

Our Form 3 theme this year is “Grow your resilience with friendship in your heart”. This theme
was chosen to reflect the importance of building strong connections and supportive relationships
in order to develop resilience to bounce back from challenges. Friendships can provide a source
of strength, encouragement, and emotional support during difficult times. In the box below,
please describe one of the challenges you have faced this year and how friendships have
strengthened your resilience to deal with the challenge.

A challenge this year and how friendships helped my resilience

I had challenges telling my class to leave the classroom when the first bell rings before CTP.
my friends helped as they were the first ones to leave the classroom when I’d tell them to
leave.

During our morning class teacher periods, we have been completing different activities to help
you bond with your classmates, practice mindfulness and be energised for the day. In the boxes
below reflect on how these morning activities are working for you.

Which activities are your favourites and why?

My favourite activity was when we wrote down our negative emotions and put it in a balloon,
that we could then pop.

How can you help your classmates during the morning activities?

I can help my classmates during morning activities by being more optimistic so that they are
not surrounded by people with low emotions.
Throughout this semester, which of these character strengths do you think you use the most?

Which of the character strengths do you currently use the most?

Leadership

How do you show this strength?

As class rep, I’ve used leaderships skills most in the mornings before CTP.

Which of the character strengths do you currently use the least?

Love of Learning

How can you further develop this strength?

I can further improve this by trying to find things I’m curious about in lessons.
PART E. Wellbeing

you to see how you are feeling and coping. Take time to reflect on your feelings in the following
