    AFFIDAVIT OF NO EMPLOYMENT
    I, MARY GRACE S. DELINA, of legal age, Filipino, single,
    and a resident of Carmen, Baguio District, Davao City,
    Philippines, after having been sworn according to law, do hereby
    depose and state that:

    1. I am the same affiant who personally and voluntary


    caused the preparation and execution of this affidavit;

    2. I was born on November 22, 1994 in Davao City;

    3. I am 24 years old now;

    4. I am not employed in the government during my term


    of office, or service requirement, and that I did not
    receive any form of salary/compensation, except
    honorarium for holding a position in an ex-officio
    capacity, during my term of office or service
    requirement as SK Chairman of Barangay Carmen,
    Baguio District, Davao City last 2010-2013;

    5. I am executing this affidavit to attest to the veracity of


    the foregoing declarations, to apprise the Civil Service
    Commission and other authorities concerned, and for
    whatever legal purpose this may serve.

    IN WITNESS WHEREOF, I have hereunto affixed my


    signature this 9th day of July 2019, in the City of Davao,
    Philippines.
