Exercise: Creating a Fillable Form in Microsoft Word

Objective:
Learn to create a fillable form in Word using the Developer tab, which includes various
types of form fields such as text boxes, checkboxes, and dropdown lists.

Instructions:
Follow the steps below to design a form that could be used as a simple survey or
registration form.

Part 1: Enable the Developer Tab


1. Open Microsoft Word.
2. Go to the File tab and select Options.
3. In the Word Options window, select Customize Ribbon.
4. Check the Developer box on the right, then click OK. The Developer tab should now
appear on your ribbon.

Part 2: Create a New Form Document


1. Start a new blank document in Word.
2. Save the document as 'Sample Fillable Form'.

Part 3: Add Form Fields

1. Form Title and Introduction


• At the top of the document, type 'Event Registration Form' (or any suitable title).
• Add a brief introduction, e.g., 'Please fill out the information below to register for the
event.'

2. Text Fields for Personal Information


• Under the Developer tab, go to the Controls section.
• Insert Plain Text Content Controls for fields like:
- First Name:
- Last Name:
- Email Address:
- Phone Number:
• Label each field appropriately and align them for a neat layout.

3. Checkboxes for Options


• Insert a section titled 'Preferences'.
• Add checkboxes for multiple-choice options. For example:
- Vegetarian Meal
- Non-Vegetarian Meal

• To insert a checkbox, click on the Check Box Content Control in the Developer tab.

4. Dropdown List for Selections


• Create a section labeled 'Select Your Session'.
• Add a dropdown list for session choices:
- Click on the Combo Box Content Control in the Developer tab.
- Enter session options, such as:
- Morning Session
- Afternoon Session
- Evening Session
- Customize the dropdown by clicking Properties in the Developer tab, then adding the list
entries.

5. Date Picker for Event Date


• Insert a Date Picker Content Control where users can select the date of the event.

Part 4: Protect the Form


1. Go to the Developer tab, select Restrict Editing.
2. Under Editing restrictions, check Allow only this type of editing in the document.
3. In the dropdown menu, select Filling in forms.
4. Click Yes, Start Enforcing Protection and set a password (optional).

Part 5: Test the Form


1. Save the document.
2. Close and reopen it to test the form fields.
3. Try filling in each section to ensure that all fields work as expected.

Part 6: Save and Share the Form


1. Save the form as a Word Template (.dotx) so it can be reused by others.
2. Share the document with a peer or instructor for feedback on usability and layout.

Bonus Tips:
• Experiment with different styles and alignments to make the form look professional.
• Add instructional text in each field by selecting Properties in the Developer tab to help
guide users on what to enter.
