1.0 PURPOSE
This procedure outlines the minimum requirements for Lock-out and Tag-out (LOTO) of energy sources that
could cause injury to persons.

It aims to ensure that equipment disconnected from sources of energy are, locked, tagged and tested for
residual energy, before work begins.

2.0 SCOPE
This procedure applies the control of energy during maintenance of equipment carried out by Engie Cofely
Mannai Facility Management (ECMFM) employees and sub-contractors within ECMFM operational control.

3.0 TERMS AND DEFINITIONS


Affected Person A person whose job requires him to be near or around an area where LOTO is
applied.

Authorized Person A person who is authorized by the Facility Manager to apply LOTO to
equipment while doing maintenance.

Energized Connected to an energy source or containing stored energy.

(Energy) Isolating Device A device that physically prevents the transmission of energy, including but not
limited to; electrical circuit breaker; disconnect switch; line valve; and any
similar device used to block or isolate energy.

Energy Source Any source of electrical, mechanical, hydraulic, pneumatic, chemical, thermal,
or other energy.

Equipment Means machinery, fixtures, structures, appliances, apparatus, and the like that
is connected to a source of energy.

Lock-out device A lock, (with accessories e.g. hasp, chain, grips), used to hold an isolating
device in the safe position and prevent the energizing of equipment. Also
included are blank flanges and bolted slip blinds for pipe ends.

Lock-out The placement of a lock-out device on an isolating device, to ensure


equipment being controlled cannot be operated until the lockout device is
removed.

Tag-out device A warning notice and a means of attachment, which can be fastened to an
isolating device.
Tag-out The placement of a tag-out device on an isolating device, to indicate that the
equipment being controlled may not be operated until the tag-out device is
removed.

Zero energy state A condition in which all energy sources in equipment are neutralized with no
possibility of unexpected start-up, or movement.

4.0 RESPONSIBILITY
4.1 Operation Managers

4.1.1 Ensure this procedure is fully implemented within their areas of responsibility.
4.1.2 Allocate adequate resources to implement the LOTO System.
4.2 Facility/Contract Manager

4.2.1 Implement the LOTO procedure and ensure it is fully adhered to.
4.2.2 Assign employees as Authorized Persons to apply LOTO during maintenance work.
4.2.3 Ensures the necessary LOTO devises are available on site.
4.3 Supervisors

4.3.1 Ensures that employees and sub-contractors are informed of and adhered to the LOTO
procedure.
4.3.2 Identify equipment to be controlled by the LOTO procedure.
4.3.3 Ensure that employees are trained and authorized to apply the LOTO procedures.
4.3.4 Issue and control LOTO hardware.
4.4 Authorized Person

4.4.1 Recognize the conditions of work that require LOTO, assess all hazardous energy sources,
and use correct procedures and materials to implement LOTO.
4.4.2 Maintain control over the key to personal locks.
4.4.3 Apply locks and tags when performing maintenance work where required.
4.5 Affected Person

4.5.1 Follow all LOTO and safety requirements.


4.5.2 Recognize when LOTO is being used, the general reasons for LOTO, and the importance of
not tampering with or removing a lock and tag.
4.6 HSEQ Manager

4.6.1 Arrange for persons to be trained in the LOTO Procedure.


4.6.2 Periodically review and revise the LOTO procedure as required.
4.7 HSE Coordinator

4.7.1 Inspect the work site to ensure the requirements of this procedure is complied with and
implemented correctly.
4.7.2 Report non-conformities and make recommendations for corrective/preventive action.
4.7.3 Take appropriate action where any conditions or practices exist that may cause imminent
danger to the safety of the persons.

5.0 EXEMPTIONS
5.1 This procedure does not apply to work on portable electrical equipment fitted with a cord and plug that
can be unplugging from the electrical energy source and where the plug is under the control of the
employee performing the maintenance work.

6.0 REQUIREMENTS
To ensure this procedure is implemented and maintained effectively, the following requirements shall be met:

6.1 General

6.1.1 A risk assessment shall be carried out of all tasks where LOTO is applied.
6.1.2 Method statements shall be prepared indicating the different energy sources and proper
sequence of isolation, for all tasks that require a LOTO.
6.1.3 Only Authorized Persons shall apply locks and tags to control hazardous energy.
6.1.4 LOTO shall be applied in conjunction with a Permit-to-Work.

6.2 Training

6.2.1 Authorized Persons shall be trained in the LOTO procedure. They must also familiarize
themselves with the method statement that shows the different energy sources and the
proper sequence of isolation.
6.2.2 Affected Persons shall be train to understand and recognize when LOTO is being used, the
general reasons for LOTO, and the importance of not tampering with or removing a lock a
tag.
6.2.3 Sub-contractor employees shall be informed that the company has and, enforces the use of
a LOTO procedure and the hazards involved with sources of energy that may be present.
6.3 Lock-out Devices

6.3.1 Facility Managers ensure suitable lock-out devices and tags are available on site. These
shall include, but not limited to lock and keys; hasps; circuit breaker grips; valve locks and
changes; wall switch covers, etc.

6.3.2 Lock-out devices and tags shall be kept in a lockable LOTO Station and only accessible to
the concerned Supervisor.

6.3.3 The spare (or master) key for each lock shall be kept with the Facility Manager.

6.3.4 Locks and keys shall be numbered. Lock shall also be color-coded to make them clearly
distinguishable for each application as follow:

6.4 Tag-out Devices

6.4.1 Tag-out device shall be readily identifiable and suitable for the environment and duration of
the tag-out.
6.4.2 All Authorized Persons installing a tag shall sign their name, contact number and expected
completion date on the tag.
6.4.3 Tags shall be used together with a durable self-locking plastic cable tie.
6.4.4 The following tags are specified for use within ECMFM.

7.0 PROCESS FOR LOCK-OUT/TAG-OUT


The following describes the steps to take to perform a LOTO:

7.1 Preparation

7.1.1 The supervisor locates and identify all sources of energy and which isolating devices to be
locked and tagged. More than one energy source (electrical, mechanical or others) may be
involved.
7.1.2 The Supervisor issue locks and tags as necessary and complete a LOTO Control Form.
7.1.3 The Supervisor informs the person doing the LOTO (Authorized Person), which isolating
devices to lock-out.
7.1.4 If persons form more than one trade (Electrical, HVAC, Mechanical), is involved in the
equipment maintenance at the same time, a lock and tag is issued for each trade.
7.1.5 The supervisor notifies all affected person that a LOTO is going to be put in place and the
reason for the LOTO.
7.2 Apply LOTO

7.2.1 Switch off the equipment to be worked on and isolate the main isolating device.
7.2.2 Place a lock on the isolating device and make sure all energy sources are locked out e.g.
valves, etc.
7.2.3 If multiple persons are involved, a person from each trade will attach a lock on the isolating
device.
7.2.4 Fill in the required information on the front and back of a Tag and place it in position with the
lock on the energy isolating device.
7.2.5 Release all stored energy from the system (such as hydraulic, air, gas, steam and water
pressure, etc.). Depending on the type of equipment, there may still be several kinds of
energy left after the equipment is turned off.
7.2.6 Verify/test to make sure the equipment is properly locked out and in a zero energy state.

7.2.7 The equipment is now locked out and tagged out.


7.3 Monitoring

7.3.1 The Authorized Person ensures the LOTO is not tampered with and remains in place during
the work.
7.3.2 The HSE Coordinator periodically inspects the work site and record results of inspections.
7.4 Restoring equipment to normal operations

7.4.1 After the task is completed and the equipment is ready for operation, clean the area, remove
all tools, equipment, and unused materials.
7.4.2 Check the area around the equipment to ensure no one is at-risk before removing the lock
and tags.
7.4.3 Notify the Supervisor that the equipment is ready to be returned to normal operation.

7.5 Removal of Authorized Employee Locks and Tags

In the event where a LOTO needs to be removed in the absence of an Authorized Employee due to
an emergency:

7.5.1 The concerned Supervisor will attempt to reach the Authorized Employee to determine if the
LOTO may be removed. If it can be, the Authorized Employee returns to the site to follow the
normal LOTO removal procedure.

7.5.2 If the Authorized Employee cannot be contacted or cannot return to the facility, the
concerned Supervisor should contact the Facility Manager to authorize removal of the
LOTO.

7.5.3 The Facility Manager ensures that no one is at-risk and it is safe to proceed before
authorizing the removal of the lock(s)
