first sworn according to law, hereby depose and state:

2. Pursuant to DILG MC No. 2001-52 dated 8 May 2001


re: rules and regulations governing the foreign travel of local
government officials and employees, an applicant requesting
to travel for purpose of travel trip or foreign trip is required
to submit, among others, a supporting sworn statement that
he has no pending criminal or administrative case in order to
be favorably recommended/authorized to travel abroad;

3. Wherefor, I am attesting to the fact that, to the best


of my knowledge and information, there is no pending
criminal or administrative case that has been filed or
pending against me before any court, tribunal or body at the
time of this application for foreign travel authority is filed;
and if there is any such case filed or pending against me, I
shall submit, together with my application for a foreign
travel authority, a clearance of a certification from the said
court, tribunal or body that I am authorized to travel abroad.
