copy);
2. One (1) recent passport-sized photo (color) with name tag
(first, middle, and last names);
3. One (1) valid government-issued ID with applicant's date of
birth (National, Passport, Postal, Driver's License, PRC, etc);
4. One (1) legal-size window envelope with Twenty-One Pesos
(Php21.00) worth of mailing stamp, and
5. Exam Fee: Four Hundred Pesos (Php400.00).

Original documents and photocopies of the


abovementioned requirements SHALL be presented by
the applicant during the processing of his/her application
at the Regional Office (RO) for purposes of validation.
Photocopies of the requirements submitted by the
applicant shall form part of the RO records.
