ASSESSMENT

1. Assess the patient’s need for nasogastric insertion.

NOTE: ALWAYS CHECK PROPER PLACEMENT OF THE TUBE


(gastric placement and sound) before and after
feeding.

3. Check patency of nares and intactness of nasal tissues.
4. Assess for gag reflex.

PLANNING
7. Wash your hands.
8. Gathers equipment.

• Plastic tube: place in basin of warm water for 10 minutes.


(large tubes)
• Rubber tube: place in a basin of ice for 10 minutes.
• Small–bore tube: insert stylet or guide wire and secure into
position, according to agency policy.
9. Plan for any assistance if necessary.
10. If weighted or special tube is to be used, review
manufacturer’s directions for insertion.
IMPLEMENTATION
11. Identify the patient.
12. Explain the procedure to the patient.
13. Place patient in high Fowler’s position with head flexed
forward. (Prepare nose skin for tape.)
14. Put a clean towel over the patient’s chest.
15. Prepare nose skin for tape.
16. Put on gloves and determine the length of tube to be
inserted (STERILE FIELD)
17. Lubricate the tube with a water-soluble lubricant.
18. Gently insert the tube on the nostril.
19. Have the patient hyperextend her neck and
advance the tube slowly aiming downward and
toward the ear.

20. After the tube passes through the nasopharynx, have the
patient flex her head toward the chest and rotate the tube 180

21. Direct the patient to sip water and swallow while you
gently but steadily advance the tube.
22. Secure the tube to bridge of the nose.
23. Check to see if the end of the tube is curled in the back of the
throat.
24. Once the tube is in, temporarily secure it with one piece of
tape and verify the tube placement if it is in
the stomach.

25. Plug the end of the tube and coil free end and pin to the
clothing
26. Make patient comfortable.
27. Give oronasal care.
28. Dispose the gloves and wash your hands.
EVALUATION
29. Evaluate using the following
criteria:
a. Patient is comfortable.
b. No irritation at nostrils.
c. Normal breathing.
d. No indications of nausea or regurgitation.
e. Tube properly placed.

DOCUMENTATION
30. Initiate an Intake and Output Record
31. Document the following on the patient’s chart.
a. Type and size of tube inserted.
b. Amount and characteristics of any drainage returned.
c. Suction pressure applied.
d. Records the results and describes the patient’s response to
the procedure.
