Notes: (Field Notes, Review Comments) Review each “Hazard” with identified safety “Controls” and determine RAC (See above)
“Probability” is the likelihood to cause an incident, near miss, or
accident and identified as: Frequent, Likely, Occasional, Seldom RAC Chart
or Unlikely.
“Severity” is the outcome/degree if an incident, near miss, or E = Extremely High
accident did occur and identified as: Catastrophic, Critical,
Marginal, or Negligible H = High Risk
Step 2: Identify the RAC (Probability/Severity) as E, H, M, or L for M = Moderate Risk
each “Hazard” on AHA. Annotate the overall highest RAC at the
top of AHA. L = Low Risk

• DO NOT CUT ANY ELECTRICAL CONDUITS
WITHOUT TESTING FOR POWER!
• Area must be barricaded with danger tape during any
demo process to prevent any worker from being hit by
falling debris, only assigned personnel to task may
cross danger tape.
• Use proper lifting techniques, ask for help if material is
too big or heavy. 50lbs WEIGHT LIMIT PER
WORKER
• Keep the work area clear and segregate debris for
better access.
• If any nails or screws are present on framing studs,
bend over or remove to prevent puncture hazards.
• Keep plenty of fire extinguishers throughout work area
(Inspect monthly), Keep water away from electric
outlets and conduits.
• Always know where all emergency exits are located.
• Drink plenty of water.
• Take brakes, if necessary, look out for each other. If
you or co-worker is feeling dizzy, or headaches make
sure to stop work and notify your supervisor
immediately.
• When Elevating scissor lift look out for overhead
structures, fixtures or utilities that can pin any part of
your body while going up. Do not place any body part
between a moving object and hard surface.
• When removing high panels make sure no one is
working below you that could by struck by debris if it
were to fall from your lift. If you have to cut any piece
from these cabinets, make sure it does not have tension
and could whiplash back towards you and hit you.
• If using Sawzall blades to cut through steel, make sure
to wear gloves when handling steel or even blades.
Steel panels and blades will be hot after cutting and
may cause burns.
