1.0 Purpose
The purpose of this method statement is to detail the procedure of capping beam construction in
accordance with the requirements specified in the relevant specification and drawings.

2.0 Scope

The following Method Statement describes the procedure, materials and equipment required for the
execution of construction of capping beam. This document should be read in conjunction with the
Project Quality Manual (where applicable) and Health & Safety Plan.

All reference made to the ‘Clients Representative” within this document shall mean the applicable
party under the terms of the Subcontract / LOA and may in instances be the Owner, the Project
Management Contractor, the Consultant Engineer or any authorized nominees as may be duly
appointed.

Details of the procedures contained herein may be reviewed periodically and modified based on the
actual sire requirements.

- Approval of work procedure Method Statement, ITP, Checklist & Risk Assessment for the work
undertaken in the project are submitted ensuring that the project works in his zone are carried out in
accordance with Company policies and in accordance with the requirements of this Quality plan.
- Ensuring the full compliance of the works with the requirements of this Quality plan.
- To ensure that all the required qualified & trained Personnel required to execute the works according
to the construction program required, and provide any additional equipment that might be
required.
- To co-ordinate with the, Project Engineer, Safety Engineer, Foreman and Surveyor for a safe and
proper execution of the works.
- To guide specific attention to all safety measures in co-ordination with the safety officer/engineer.
- Ensure area is ready and safe to start the works.
- Set up required equipment and plant through discussion with the Project Engineer/Works Supervisor.
- Ensure the works are carried out according to the specification, quality and approved shop
drawings.
- Liaise and co-ordinate with the Project Engineer for the agreed sequence of works with respect to
the construction programmed.
- Allocation of required manpower through co-ordination with the Construction Manager.
- Provide the risk assessments for the works in hand.
- Provide sufficient and safe access for operatives, trucks and pumps.
- Take precautionary measures with regards to protecting works from hot weather, wind, rain & sun.
- Responsible towards implementation of Project Quality Plans & HSE Plans requirements.

6.5.2 Project Engineer

- Oversee and direct construction projects from conception to completion


Review the project in-depth to schedule deliverables and estimate costs
- Oversee all onsite and offsite constructions to monitor compliance with building and safety
regulations
- Coordinate and direct construction workers and subcontractors
- Select tools, materials and equipment and track inventory
- Meet contractual conditions of performance
- Review the work progress on daily basis
- Prepare internal and external reports pertaining to job status
- Plan ahead to prevent problems and resolve any emerging ones
- Analyze, manage and mitigate risks.
- Ensure quality construction standards and the use of proper construction techniques
- The engineer shall carry out his duties in a manner that shall be coordinated by the project manager
daily and shall ensure proper distribution of the workforce and equipment at required locations.
- To co-ordinate with the Safety Officer to maintain safe working and proper housekeeping of the site.
To comply with the safety measures and ensure that all Safety Officer Teams are aware of the same
to prevent accident and loss.
- To monitor and check all activities and ensure that works shall be carried out according to
specifications, quality and approved drawings.
- To raise the Inspection requests in a timely manner.

6.5.3 HSE Officer


- To ensure implementation of all safety measures related to the nature of works being carried out,
and in accordance with the Project HSE Safety Plan & Method Statement, Risk Assessment etc.
- To ensure that all the persons involved in the works are aware of their responsibilities, and that they
have enough understanding of the safety procedures.
- The safety officer in co-ordination with the Project Manager shall ensure that all the implemented
safety measures are effective enough to maintain safe working on the site.
- To maintain continuous inspections of the site activities, advise and train persons daily to prevent
accidents and personnel injury.
- To give special concern to housekeeping and ensure that the site is maintained clean and tidy.
- To ensure all the relevant safety sign boards for different works are in place.

6.5.6 Site Foreman


- Ensure the works are progressed in the sequence as agreed with the Project Engineer.
- Liaise with the Project / Construction Manager /Lab Agency /QA/QC department for the allocation
of the work force, ensuring adequate manpower and resources is available.
- Liaise with the Project engineer to ensure all the required plant / materials are available to execute
the works.
- Full time supervision shall be required to ensure the works are carried out in accordance to
specifications, quality and approved shop drawings.

6.5.7 Surveyor

- To establish benchmarks from agreed reference points, provide required setting out and level
markings and follow up with regular checks as per the approved test pile location
- Co-ordinate with the Project Engineer / Foreman and ensure the approved test pile location
drawings shall be implemented properly.
- Maintain survey details and reports, periodically check the progressing works and advise the Project
Manager /SPML of any deviation from the drawings.

6.5.9 Heavy Equipment Operator

- The Heavy Equipment Operator's duties include preparing and manipulating heavy equipment to
ensure the appropriate movement of materials, and liaising with coworkers to ensure that the
projects are completed accurately. You will be required to conduct your duties in a safe, diligent
manner at all times.
- Ensure to monitor workflow, suggesting alterations to enhance productivity, as needed.
- Ensure the attuned to the subtleties of each situation, possessing sound knowledge of when to lead
or follow.

6.5.10 Rigger

- Inspecting and preparing loads that need to be moved.


- Setting, aligning, and leveling heavy equipment machinery.
- Selecting the appropriate rigging gear.
- Preparing rigging equipment including beams, pulleys, clamps, and bolts.
- Inspecting rigging before final use.
- Monitoring and maintaining rigging equipment.
- Maneuvering loads using heavy equipment machinery and by hand.
- Ensuring compliance with company safety procedures.
- Communicating with the operator and construction team.

7.0 Safety Precautions


-Strike to reduce the probability of the accidents which have the potential to cause injury,
disablement, loss of life & property
-To educate the employees so that they can be aware of their own safety, health & wellbeing as well
as their responsibility towards the environment.
-To train, retrain & thereby motivate the employees so that they are able to identify & eliminate
prevailing unsafe practices as well as we able to improve, upgrade & mention their workplace free of
unsafe action & condition.

7.1.1 Health & Safety (Basic Safety Rules)


- All workers are must to wear cover all uniform, helmet and hand gloves while on duty.
- Do not start any work without a proper PPE.
- Smoking is strictly prohibited in plant.
- Safety harness (if personnel are working on height)
- Construction team Project Manager and Site Engineers shall proactive to make sure coordination with
the safety officer in compliance with the HSE procedure.
- Work place and surrounding area shall be kept clean and free from obstructions and site safety
induction will be conducted and also Toll Box talk meeting is implemented prior to any activity to start.

7.1.2 Welfare

Suitable welfare facilities will be provided from the start and are maintained throughout the
construction phase.

Eating and Drinking Areas (Workers/ Staff):


Employees may not consume foods or beverages in any area where there may be toxic or harmful
materials. All employees shall consume food only designated welfare area. The eating area shall be
kept clean and hygienic all the time. The eating area shall be provided with air condition facility with
adequate fire extinguishers.

Water Facilities:
The following shall apply to Drinking Water (Potable): All containers shall be clearly marked as
“DRINKING WATER” and not used for any other purpose. Water shall be supplied in adequate amounts
in tightly closed containers equipped with a tap (No dipping allowed); At least 5 litre (not limited to) of
drinking water per person shall be maintained; A cool drinking water station will be also provided.

Rest/Eating area
An Air-condition rest area/eating area for workers for taking meals will be provided. The rest area will
be also equipped with chairs and tables. If site work force will increase at site rest/eating area will be
provided adequately.
The rest/ eating areas shall be kept in a clean hygienic and orderly state at all times and shall not be
used for storage of material or equipment. A cleaner will be provided to clean and maintain
rest/eating area. Food waste will be disposing on regular basis.

7.1.3 Waste Arrangement

Waste skips shall be deployed at the area in order the general waste to be disposed. The hazardous
waste shall be deployed separately in a hazardous waste area. Waste area shall be removed and
pumped out as required by an approved contractor. Approved contractor to be appointed to dispose
and replace containers.

7.1.4 Night Time (if required)

- PPE like goggles, face mask, apron (visible jackets), gloves, safety shoes, helmet etc. are issued for
personnel protection for personnel while engaged on such work.
- Proper lightings in the area of work.
- Constant checking for the additional requirements in the area of work.
- Site engineer and HSE officer must be available on site and must be made aware of all emergency
procedures. An emergency vehicle shall be available on site at all time.

7.1.5 Dust Control

-In case of high dust level, dust must be suppressed by sprinkling water. Personal exposed to dust shall
wear dust masks.

7.1.6 Health Environmental & Safety Provision

- The work shall be carried out in accordance with the approved project safety plan.
- Regulated with safety requirements from our construction team safety officer prior the start of work
- All works shall be governed according to project safety plan.
- Emergency First aid kit shall be available in the area.
- Supervise to oversee by ta qualified safety officer for the improvement of safety matter as require and
can implement safety decision for such activity as needed.
- All staff/personnel shall wear all PEE in all times during the construction activities.

7.1.7 Emergency Procedures


Procedures will be followed.
To ensure that all workplaces plan for potential emergencies to provide effective management should
one occur. This includes the preparation, issue, control and updating of an Emergency Response Plan
(ERP) plus the training associated with maintaining readiness for an effective Emergency Response. It
also provides guidance for dealing with traumatic injury or death and the reinstatement of controls
and systems to ensure a timely return to normal business.
This procedure applies to any event that threatens life, property or the environment related to any
Dutch project or site, including Joint Ventures and subcontractors.
Emergency: A significant event that threatens life, property or the environment.

Site/Project Manager Shall:

Prepare, review, authorize and distribute a controlled copy of the Emergency Response Plan (ERP)
to all personnel as appropriate.

Appoint a responsible officer and deputies to the emergency team and assign responsibilities for
activating the emergency response procedures.

Ensure all employees receive training regarding site specific emergency procedures.

Ensure the ERP plan is reviewed and updated annually as a minimum, or:

Following any changes to the workplace that introduces new potential emergencies to the site or
which may alter the intended execution of the plan. For example, exit routes, alarm systems etc.
Where any incident debriefs or training exercise identifies an opportunity for improvement.

Purpose
The purpose of this procedure is to detail the process for responding to Medical Emergencies and First
Aid on Dutch International Projects. Project responses exist specifically to deal with major incidents
impacting the safety of the personnel and the environment.

The priority where an incident, injury or near miss occurs is:


- Care of injured persons; and
- Making the area safe to prevent any further injury or property damage.

The Project Manager, Project Engineer and the Project Safety Officer are to be notified immediately of
any significant injury or near miss.
The Site Foreman and Project Safety Officer or the other responsible employer’s representative will
notify the injured employee’s family as appropriate.
The Safety Manager must be notified immediately of any site medical treatment or significant near miss
incident.

8.0 Methodology / Detailed Procedure

8.3 Excavation & DW head cutting

 Excavation shall expose the shoring wall and area for working platform.
 Setting out for the excavation line, width, depth and other dimensional requirements shall
be carried out by the surveyor in accordance with the approved drawing.
 Excavated material shall not be piled up /stocked along sides of excavations in a manner
that will over load or increase danger of collapse of excavated sides.
 After excavation has been completed and reach the final required level, reference point
will be marked.
 The surveyor shall mark the shoring wall cut off level.
 After the cut-off-level has been marked on the shoring wall, a handheld Pneumatic Jack
hammer mounted with compressor shall be used to break the concrete portion above the
shoring wall cut-off level or Jack hammer mounted to excavator.
 Final chipping to be carried out using a handheld electric concrete breaker that will
enable finishing the shoring wall top to the allowable tolerance from design cut-off level.

8.4 Reinforcement

 The concrete reinforcing steel bars will be obtained from an approved source and conform to the
specified manufacturing standards.
 The material will be delivered to site cut and bend cold with diameters, shapes and sizes
conforming to those given on the bending schedules based on the approved shop drawing.
including the required lap length.
 Most reinforcement will be fabricated on site, as 12-meter steel cages and shifted to the required
location.
 All reinforcement shall be stored free off the ground, well covered and before installation shall be
free of oil, grease, loose rust or other deleterious matter.
 Reinforcement bars shall then be shifted to the working location by mobile crane and fixed to the
accuracy shown on approved shop drawing with respect to number, diameter, spacing and cover
by the use of approved binding wires ties and spacer blocks.
 Ensure that fixing is firm enough to prevent rebar displacement during the concrete placing.
 All the bars shall be protected from weather, dust and humidity by covering with tarpaulins.
 Steel fabrication shall be carried as per the approved design drawings. All the anchorages
reservations shall be in as per the approved design drawings.
 Lap length shall be as per specified in drawings.

8.5 Formwork

 Forms shall be placed, aligned and secured by necessary propping, providing safe and stable
working platforms for checking and concreting operation.
 Ensure all formwork joints are suitably sealed prior to closing to prevent grout leakage.
 The formwork system should have the minimum number of elements in order to facilitate fast
erection and striking times. Pre-fabricated steel beams will be used for the formwork.
 The formwork shall be formed using H-beams and braced as necessary. The height of the formwork
shall600mm or more. Before fixing the framework, it will be inspected for any defects, and any
damaged will be replaced as required.
 The formwork shall be arranged as to be readily dismantled and removed from cast concrete
without any shock, disturbance or damage and as per project specification.

8.6 Prior to concrete placement

 Structural concrete works shall commence upon completion of reinforcement and formwork
activities.
 WIR to be submitted.
 Concrete should be deposited at or near final position to avoid segregation due to lateral flow.
 Layers of concrete shall be integrated with previous layers by re-vibrate the pre- layer.
 All the surfaces shall be cleaned. Foreign matters such as saw dust, wooden and wire pieces shall
be removed.
 All the joints shall be water tight to prevent the loss of grout.
 Surfaces of the formworks shall be kept moist; standing water shall be removed before concreting.
 All the chutes, buckets, pokers and hand tools shall be cleaned with water before and after
concreting.
 Construction joints are formed when concreting is interrupted unexpectedly. Shop drawings
showing the location of construction joints.
 All necessary spacers shall be provided to maintain the specified covers.
 After completing the concreting all the waste concrete shall be removed.
 Where reservation pipe (For Inclinometer) is protruding through this pour, that are not to receive
structural concrete at this pour, they will be protected from concrete splash by wrapping with
polythene sheet, or other suitable material.
 Required anchors, sleeves, dowel bars and other inserts shall be provided as per the drawings.
 It shall be ensured that enough stand by vibrators; pokers, hand tools, hessian clothes and
polythene sheets are available.

8.7 Frequency of sampling for testing

 Prior to concrete pouring fresh concrete testing shall be done in site concrete slump, concrete
temperature and ambient in the designated concrete checkpoint.
 Concrete temperature shall be not more than 32 degree Celsius and ambient temperature shall
not be more than 39 degrees Celsius at least.
 Slump shall comply from the approved mix design.
 Compressive strength shall be test for as per the project specification for 7 days and 28 days.

8.8 Placement procedure for concrete

 Concrete placement sequence shall be planned in a manner so that to have minimum


repositioning of the transit mixer /concrete pump/crane.
 During hot weather conditions precautions/ preventive measures shall be taken as applicable/
required to control the effect of high ambient temperature, solar radiation and low relative
humidity.
 Chilled water shall be used for batching in mixing concrete in the patching plant.
 The delivery ends of the chutes or the pump hoses shall kept as close as possible to the delivery
points. The chutes shall be placed in suitable slope to avoid flashing and segregation.
 The concrete in each section of structures between joints or corners shall be placed in successive
pours, working away in both directions from the center most of the section.
 Placing of structural concrete will generally be by, crane skip, directly from the vehicle or pump (if
required) to ensuring satisfactory distribution by control of rate of discharge and movement of the
chute.
 Concrete will be deposited continuously or, where this is impractical, at such times as will ensure
the previous layer is still in a plastic state and cold joints will be avoided.

 The concrete will arrive at the site and be approved by the Consultant in charge to verify concrete
class.
 The concrete truck mixer will discharge the fresh concrete directly from the vehicle or pump into
the required location.
 Falling of concrete over reinforcement or in a manner which may cause segregation shall be
avoided.
 The capping beam will be casted in stretches of 12 - 15 meters.

8.9 Compaction/Vibrator

 Trained operators will be assigned to this activity.


 The number and size of vibrators shall be complied to ensure thorough compaction of the entire
volume of concrete poured.
 Vibrators shall be operated at regular intervals to produce dense, compact, homogeneous
concrete surface.
 The size of the vibrating pokers shall be decided considering the layout of reinforcement and shall
ensure consistent mass is obtained without segregation.
 The diameter of the vibrator needle shall be 20 mm to 60mm depending on the spacing and
layout of the reinforcement and shall ensure thorough compaction of the entire volume of the
concrete.
 Vibrators shall be withdrawn slowly to prevent the formation of voids and shall not be placed
against compacted concrete.
 The concrete will be compacted using vibrating pokers of adequate size and number.

8.10 Finishing

 Hand trowel will be used for the surface finishing.


 Competent mason will be deployed for the required work.
 Finishing as per project requirements.

8.11 Striking of formworks

 Striking will commence after removal of support system or backfill material.


 Suitable crawler carne will be used for the removal of the steel beam formwork.
 Where if required, any repairing works to the concrete surface shall be performed using approved
repairing material under the discretion of the consultant and prior to proceeding with further
activities.
 After the removal of the formwork, it will be inspected and cleaned.
 Striking/ dismantling should be as per project specification.

8.12 Curing

 After the concrete hardens the polythene sheets shall be removed and replaced with wet hessian
cloth shall be used to the required coverage.
 Water may use for curing and shall be free from salt, harmful matters and other impurities.
 Wet hessian cloth shall be kept damp continuously during curing period to ensure that all faces of
concrete surface are wet.
 Concrete shall be cured by water as per project requirements and specification.
