Report Defects Procedure
1.0 OBJECTIVE
1.1 This Procedure specifies the steps to be followed when reporting break downs.
2.0 SCOPE
This Procedure specifies the reporting of break downs within the transport
department and this will be the break downs within the trucks, buses, small vehicles
and earthmoving machinery.
3.0 RESPONSIBILITY
3.1 The Transport & Services Manager shall maintain and ensure the implementation of
this procedure.
3.2 The Transport Foreman shall implement this procedure

5.0 DEFINITIONS
5.1 The “company” – means Zimplats.
5.2 DDC – Defensive Driving Certificate, being a certificate that is issued by the Traffic
Safety Council of Zimbabwe in conjunction with the Zimplats Training Manager, after
the driver has successfully completed the relevant course of instruction.
5.3 PPE: Personal Protective Equipment.
5.4 Internal drivers’ license: The license issued by the company after having passed
the driving test conducted by the transport foreman
7.0 REQUIREMENTS
7.1 PPE Required

7.1.3 Gloves
7.1.4 Hard hat
7.1.5 Reflective vest
7.1.6 Goggles
7.1.7 Dust mast
7.3 No Go Issues
The vehicle must:
i. Have a spare wheel, jack and wheel spanner
ii. Have a First Aid Box
iii. Breakdown triangle x 2
iv. Carry a Log Book at all times
v. Vehicle check list.
8. PROCEDURE
1) The driver or the operator reports the break down or defects to the transport
supervisor.
2) The transport supervisor reports to the transport foreman.
3) The transport foreman instructs the transport clerk to raise the break down
notification.
4) The transport clerk gives the notification to the transport foreman.
5) The transport foreman then gives the notification to responsible mechanic/diesel
plant fitter to rectify the problem and note down.
6) The motor mech or the diesel plant fitter rectifies the problem.
7) Should the problem require out side attention:
NB: Refer to vehicle sending out for repairs procedure and check the
process required.
In case of the earthmoving machinery the transport foreman must contact
the service provider to come on sight to rectify the problem.
The transport foreman the must make a follow up for the vehicles send out
for repairs.
8) In case of the motor mechanic/diesel plant fitter having rectified the problem,
he/she must road test the machinery.
9) If satisfied that everything is ok then the mechanic reports to the foreman.
10) Then transport foreman then surrender the vehicle, truck or bus to the transport
supervisor.
