Please submit electronically all of the listed completed documents with the checklist to
University Relations at [email protected].

University Relations staff will acknowledge receipt and conduct a review of the submitted
documents to verify that all eligibility requirements of the Affiliation Program have been met
and will notify the institution of the outcome of their application.
