Terms & Conditions

This Service Order is partial of Service Agreement with terms and conditions mentioned as follows:

➢ Customer shall inform prior (30) days for termination of the plan.

➢ Payment method is postpaid.

➢ All price are exclusive 5% commercial tax.

➢ Payment has to be made by the customer within 15 days after the invoice issued date. Otherwise the
order shall be terminated.
➢ For SMS remaining of each month: will be used within 3months. After 3 months, the SMS will be
deleted. All benefits are forfeited at the end of 3 months cycle.

The customer hereby agrees and accept terms and conditions of Mytel and pay for the service fees as described hereto
and subject to the term of payment. Service Agreement is made on the date of the later of the two signatures below:
