    AFFIDAVIT

    THIS AFFIDAVIT is made at Mumbai

    I, MRS. ANISHA DEEPAK SARANG, Adult , Indian inhabitant,


    residing at Ganesh Nagar, Ganesh Galli, Near Ganesh Mandir, Salt Pen
    Road, Wadala (East), Mumbai – 400037, do hereby solemnly affirm
    and declare as under:

    1. That my name daughter Ms. ------------------------------------- is a
    student of RANM 1st year September Batch-2023 at your
    reputed institute at Cama & Albless Hospital, Mumbai.

    2. I further state that I am willing to send my daughter for Hospital


    Clinical Experience Internship at Ulhasnagar No.4 Hospital on
    my risk and responsibility.

    3. I further state that it would be difficult for my daughter to travel


    back home on daily basis after working hours at the hospital
    therefore, I intend to let my daughter reside at a rental premises
    at Ulhasnagar itself.

    4. I further state that my daughter along with her friends would


    reside at Sitaram nagar, Ramnagar Road, Maratha Section 32,
    Ulhasnagar No.4 and further state that i am aware of the same
    and have given my consent and consideration to reside there till
    the completion of the internship of 2 months from 27/05/2024 to
    21/07/2024.

    5. I further state that i would be fully responsible for any instance


    caused during the time of internship programme and would not
    blame the institute for any kind of mishap or emergency
    situation that may occur during the course of time.
