bunker two times more on 09 September 2020 and 12 September 2020, the bunker analysis results were not
available on-board and the bunker had already been in use. As per the records, bunker samples were landed for
analysis.

Inspector Observations: Lube oil analysis report was noted in red for Emergency generator cranckcase cylinder,
report date 23 Jan 2021, sample date 20 Dec 2020 LO analysis by Gulf quote Drop in flashpoint and viscosity is
together indicative of the fuel ingress into lube oil. The oil is not suitable for further use provided the landed
used oil sample is representative for the applications. Fuel ingress source shall be identified and appropriate
corrective measures take. After eliminating the the cause of contamination oil change should be renewed using
the correct grade. unquote

winches, cargo pump diesel engines were required to be tested every 6 months and tested on 29 March 2021.
The analysis result of steering gears, cargo diesel engines, mooring winches, hose crane were not sighted during
the inspection. As per the crew advise these analysis results were not sent to the vessel.

Inspector Observations: Noted that the testing of lube oil analysis (3 monthly) for main engine, diesel
generators and stern tube were overdue. Latest analysis report available on board was dated 1 Feb 2022.

Inspector Observations: Last lube oil analysis report for remote control valve system dated 13 Nov 2021 was
rated 'Critical' due to high particles and viscosity.

Inspector Observations: BNWAS alarm, audible alarm provided on the reset button on either side of the bridge
wing was not operational when tried out. Only the visual light indication for the alarm was operational. Note:
During inspection, port side bridge wing audible alarm on the BNWAS reset button was rectified

Inspector Observations: The main engine RPM indicator installed on the starboard bridge wing was out of
order. A notice was posted on the indicator, stating that the status was out of order and same was confirmed by
the ship staff.

Engine telegraph printer / logger located on navigaLonal bridge was found out of order.
The GPS no. 1 on 28 Dec 2022 was displaying date as 14 May 2003.
The VDR panel located on the Nav.Bridge had following alarm 6047 (echo sounder signal)
Inspector Observations: Several deck structures, supports, posts, clamps and flanges on main deck noted
moderately corroded. It was observed that maintenance of the main deck was in progress and completed from
aft main deck to No.4 cargo tanks.
Inspector Observations: The main deck, deck pipelines flange connections, supporting elements and water
ballast tank manhole covers cosmetic condition was observed with scattered severe rust spots. Maintenance
was in progress from the aft to the forward.
Inspector Observations: Various spot / localized, moderate to hard rust were observed on the main deck and
transverse, longitudinal elements of main deck and poop deck.
Inspector Observations: Deck was moderately rusty partially from aft accommodation start point till manifold
area in coamlings.

Inspector Observations: Following Statutory finding was noted in the latest Class status report dated 21 Sep 2020. The finding
(ID No. SF-I-001) was imposed on 04 Aug 2020 and due by 05 Oct 2020. Description: As per the Panama Flag Administration's
authorization, a short term IOPP Certificate (Form 2222) was issued valid until 05 October 2020, pending installation and
verification of final approved stability instrument on board in accordance with regulation 28(6).

Inspector Observations: The class survey status report dated 18 December 2020 sighted. There was 01 Statutory findings: 1.
A conditional IOPP Certificate issued valid till 28 February 2021. Pending installation & verification of final approved stability
instrument onboard. And 01 Actionable item imposed on 14 March 2019 with compliance date due on 31 December 2020
i.e. to have on board Inventory of Hazardous Material inventory and certification as per EU regulation 1257/2013. This
actionable item to be deleted when completed.

Inspector Observations: Below Actionable item was noted on class Status report AI-S-001 Imposed 18 Dec 2020 Due date 31
Dec 2025 Category - Statutory On or after 31 December 2020, vessels calling any EU port or anchorage and/or flagged by EU
member state shall have on-board a verified inventory of Hazardous materials (IHM)and certification in accordance with the
EU ship recycling regulation 1257/2013. Actionable item to be deleted when completed.

Inspector Observations: CC 5, CA 609, MO 27, RR 851, RS 852, class status report dated 04 Jul 2021. CA 609 Issued 2021-03-
15 Issued at Cape Town Due date 2021-06-15 New SOPEP/SMPEP, SEEMP part II to be send to DNV for approval. MO 27
Issued 2021-03-15 Issued at Register and Data Management Machinery planned maintenance system (MPMS) Since last
MPM survey the vessel has changed manager. If the vessel shall remain on MPMS, the following shall be considered: 1. PMS
software installed to be approved by DNV2. Next MPMS annual survey shall be with scope of a MPMS initial survey in order
to verify correct implementation of the software. Inform using DATE if the vessel shall not continue with MPMS. The vessel
will be changed to machinery continuous survey arrangement, and due dates for machinery items will be 5 years from last
MPMS annual survey. The attending surveyor shall 'delete' this MO latest at next MPMS annual survey. MO 28 Issued 2021-
07-01 Issued at Technical support The vessel's name was painted pending the permanent marking.

Inspector Observations: The following Memo (MO 12) was applied to the vessel by Class on 30 August 2015: "Water ballast
tank 3 Starboard minor indentation in shell at frame 81-82, side longitudinal 40 affected."

Inspector Observations: All the deck and engineer officers including Master and Chief officer, Second officer and
Third officer, Chief engineer and Second engineer, Third engineer and fourth engineer joined the vessel at
Gibraltar on the 14th August 2020. OCIMF recommendations were that Master & Chief officer and Chief
engineer & Second engineer are not to be relieved at the same time

Inspector Observations: The current operator assumed responsibility of the vessel with onboard crew on 03
September 2020. The on-board crew continued with the new technical operator. The downloaded Officer
Matrix dated 15 September 2020 was showing that the deck and engine officers had been working (duration 0.4
years to 1.5 years for different ranks) with the new operator. However, they had been working with the new
operator only since 03 September 2020.
Inspector Observations: Due to COVID-19 travel restrictions, noted that the Master & Chief Officer had joined
the vessel on 27 Apr 2022 at Sohar, Oman and were relieved at the same time.
Inspector Observations: Vessel's port of Registry was Malakal Harbour and flag was of Republic of Palau. Officer
matrix showed for all officers, certificate issuing country as 'Turkey'.
Inspector Observations: Due to COVID-19 travel restrictions, the Master and Chief Officer had joined the vessel
on 8 Oct 2021 and were relieved at the same time. Also, noted that the application for Djibouti certificate of
endorsement for 3/officer (applied on 16 June 2021) and C/Engineer (applied on 2 Nov 2020) were still under
application.
