ACCOMPLISHMENT REPORT

Rural Health Unit 1

 June 03, 2024


 Attended the flag ceremony at Municipal ground around 8:00 to 8:30
 We had an orientation regarding to our designation assign, and I was assign in RHU 1
 After they assign the designation to us, we immediately go to the RHU1 and we are just
3 assign here and we're assign in the information desk
 Ma'am Jocelyn Gonzalgo is one of our head here in RHU1 and she gave me something
to write where in it is a referral of the patient
 June 04, 2024
 I just continue the task that was given to me yesterday
 Assisting some patient to log book if they will be having their check up or medical
 We clean some papers in the cabinet and arrange it in order
 June 05,2024
 This day is same with yesterday, we are assisting the patient if they will be having check
up or medical
 After the checkup of the patient with Dr. Van or Dr. Payoyo, they will to us the referral
and we will encode it
 June 06, 2024
 This 4th day, we assist again the patient if they will be having their checkup or medical
 June 07,2024
 This day i wiped first the desk before continuing the task that was given to me

 For the 2nd week of our summer job, we attended first the flag raising ceremony in the
municipal ground
 We clean first our surroundings before doing our task
 We assist some patient with their check up and regarding with their medical

 Today we don't have an attending Doctor and That makes us Not Loaded. We just assist
some patients to Go to Laboratory, dental, and Nurses station, and Some are advised to
Go back next day When Doctors are On Duty for their Check-up and Medical.

 Assisting some Patients to Logbook and Helped them with their referral and with their
medical

 For the 3rd week of our summer job, We Attended the flag ceremony at the municipal
ground
 Also Today we don't have an attending Doctor and That makes us Not Loaded. We just
assisted patients to Go to Laboratory, dental, and Nurses station and Some are advised
to Go back next day When Doctors are On Duty for their Check-up and Medical.

 We are just half today because in the afternoon the doctor’s and some nurses they have
