    AFFIDAVIT

    I, VEE ANN VILLANUEVA AFABLE, of legal age, Filipino, single and a


    resident of KG8-643 PNR Compound, Barangay 10, Lucena City, having been
    sworn to in accordance with law, do hereby depose and state that:

    1. I am presently residing at KG8-643 PNR Compound, Barangay 10,


    Lucena City;

    2. I am not the owner of the land in the aforementioned address;

    3. I am applying for reconnection of my electricity in the said address;

    4. I undertake that in the event that the lawful and/or absolute owner
    of the parcel of land where I reside is going to use the same, I will
    freely and voluntarily vacate it;

    5. This affidavit is being executed as a requirement in reconnection of


    my electricity;

    6. I am executing this affidavit to attest to the truthfulness of the


    foregoing and for whatever legal purpose it may serve.
