7 DEVICE DESCRIPTION
7.1 INTRODUCTION.
The RTP system is a machine that can accept images from a video camera, process
and memorize them on a magnetic support (hard disk). The system is available in
two versions - MR and HR – with the different types of acquisition hardware.
RTP is a system which allows transfer of imported images and related information to
TCP/IP networks with DICOM 3 protocol.
Depending on RTP configuration, imported exams can be sent to printing devices (a
PRINT SERVER or printer connected with the network) or to archive systems
(IMAGE SERVERS).
Optical disks (CD/DVD-ROM) can also be produced in order to review patients’
exams on computers using the DICOM Viewer program recorded on each CD/DVD-
ROM produced.
7.1.1 MR System.
The PPR-04 card pre-processes analogical video signals from the 625-line CCD
interconnected camera. The 12-bit digitalized video signal is processed by a
dedicated digital circuit with the following functions:
• Anti-vignetting
• Auto offset
• Recursive filter
• Recursive filter with movement detection (smart filter)
The digital circuit also interfaces with the control panel as:
• Signal input/output to and from the control panel
• Infrared remote control
7.1.2 HR System.
The PPR-05 card pre-processes analogical video signals from a progressive 1024-line
CCD camera. The 10 to 12-bit digitalized video signal is processed by a dedicated
digital circuit with the following functions:
• Anti-vignetting
• Auto offset
• Recursive filter
• Recursive filter with movement detection (smart filter)
The digital circuit also interfaces with the control panel as:
• Signal input/output to and from the control panel
• Infrared remote control.

7.2 ALGORITHMS
In logical sequence the algorithms applied at the original data of the images are the
following:

NOISE REDUCTION: the fluoroscopic images made at low dose are unfortunately
characterized by a high amplitude of the noise that clearly affect in part the visibility
and the perception of the operator. The usual process now used by all manufacturers
of vision systems for radiological application is also called temporal averaging filter
(average between successive images) also proposed by the manufacturers of RX
sensors and cameras. It also appears in the scientific literature for the reduction of
noise in all applications with high noise content (radiological, astronomy, military,
etc..)

DIGITAL ROTATION: to allow an easy visualization as a function of position of the

implant relative to the patient table applies the digital electronic rotation followed by
a bilinear interpolation algorithm for elimination of artifacts.

DIGITAL SUBTRACTION: point-to-point operation that performs the arithmetic

difference between the corresponding pixels of two different images; the first,
normally defined as "mask", stored before the introduction of contrast medium, the
second image defined as post-introduction of the contrast medium .

CONTRAST STRETCHING: an operation that improves the appearance of the

image improving the contrast, is the contrast stretching algorithm which allows to use
the whole dynamic of the visualization tool (in our case, the monitor), this algorithm
can be applied automatically in real time, or by allowing the adjustment at the
discretion of the operator. The application of a curve of logarithmic or exponential
allows to highlight the contrasts in the parts with higher or lower density
radiographic.

EDGE ENHANCEMENT: algorithm that allows a local place greater emphasis on

the details of the contours of the image by applying a local array with suitably
calibrated coefficients applied to every pixel and its surrounding pixels.

ZOOM: algorithm was applied only in a local post-processing, digital magnifier with
bilinear interpolation to reduce the effect of discretization of individual pixels.

SERVER NOTIFICATION CONDITIONS

In this section the events that cause a message to be sent to the server are selected.
The sending is secondary to the general enabling (Enable Notification from Server)
and the presence of an active MPPS.
The check boxes are divided into two columns: N-CREATE and N-SET. If boxes are
checked in the N-CREATE column, an N-CREATE command is sent from the MPPS
when the described event takes place. If boxes are checked in the N-SET column, an
N-SET command is sent from the MPPS when the described event takes place; but
first the N-CREATE command must have been successfully sent following an event
that calls for it.

On open study if
MPPS already exists: when a study is opened, with the added condition
that the active MPPS object must have been
created during an opening before the study.

On new image acquisition: end of acquisition of a new image

Before image DICOM transfer: instant before a Dicom Store.

After image DICOM transfer: end of a Dicom Store. Note that although
with Dicom Print and Dicom CD Write
images can be marked as “Dicom transfer”,
they are never followed by MPPS messages
to the server.

After a Stg Commit.: end of the SC of an image (predisposition for future

implementation).

On image modification: modification of an image that has already been

Dicom transferred.

On closing a study
(N-CREATE + N-SET: in this case the condition is also part of the
N-CREATE message if this has not been done.
The check box also is also used to generally enable.

11/06/13
RTP_uk_technical manual_WXP_v115x_Rev5.2.doc Page 58 of 153

ON CLOSE STUDY N-SET NOTIFICATIONS

In this section, the send details of the N-SET message are configured (preceded by N-
CREATE if necessary) at the moment a study is closed, always if the relative check
box in the previous section has been selected.
At study closure the N-SET command can be transmitted automatically or with the
confirmation of the operator.

Always automatically: N-SET is always sent automatically.

Automatically if new
acquired images: Automatically sent if the active MPPS contains
links to undeleted images whose UID is that created
at the moment of acquisition (therefore not
regenerated after Dicom transfer and successive
modification of the image).

Automatically if DICOM
transferred images : Automatically sent if the active MPPS contains
images marked as “Dicom transferred”.

Automatic if MPPS already

created on Server: If the active MPPS has already been created on
the server (send of N-CREATE), it is automatically
completed.

Ask operator if no
Auto conditions: If N-SET has not been automatically
sent, an active MPPS exists and it contains links to

STORAGE COMMITMENT SETUP

The Storage Commitment Dicom service (henceforth refered to as SC) allows a

Dicom device that has transfered images to another device (normally a workstation or
an archive) to entrust the second device with the permanent storage of the images.
Upon receiving notification of acceptance, the first device can then do what it wants
with the images, including deleting them.
SC takes place between two devices that have the roles of SCU and SCP.

SCU is the device that sent the images and requests commitment.
SCU carries out a N-ACTION, that is a request for SC service.

SCP is the server of the SC and has received the images for which it accepts
commitment.
SCP sends a N-EVENT-REPORT, that is to say a confirmation of the execution of
the SC command.

12 SYSTEM TEST.
The final system test is a complete check of the RTP hardware as described below. It
is divided into 3 steps: Cables and Connectors Checks, Electrical and Functional
Check, and Report.
12.1 CABLES AND CONNECTORS CHECKS.
Visual inspection of each single cable and connector to identify possible fractures or
damages:
A.1 RTP Power Cable and Power Connector Check.
A.3 Ethernet Connection Check (RTP to DICOM Network).
12.2 ELECTRICAL AND FUNCTIONAL CHECK.
Test transmission of one image in the DICOM network to verify that the RTP is fully
operational.
Note: Periodic repetitions the tests above ensure higher system reliability and
longer operational life.
12.3 REPORT.
Users are responsible for filling out a Test Report:
- during routine maintenance
- after any special maintenance.

Each check has a specific alphanumeric code-tag with reference to the test outline
and to the corresponding brief description in the chapters above.

Code Check Result Note

B.1 Test Image Transmission in the DICOM Network

12.4 MEMORY CLEAN-UP.

Exams already imported by the RTP may fail to be transmitted to the DICOM device
due to problems with the connection to the network equipment.
These exams are still stored on the RTP hard disk and occupy some memory space.
Problems occur only if the spooler settings do not enable the automatic deletion of
images that could not be sent out. In this case periodical checks are required to
identify possible failed transmissions and manually delete unsent exams. To delete
them, select DICOM SPOOLER from the Task Bar, to open the following window:

on “Disable Queue” and “Disable Spooler”. The fields in the dialog window will
change to “Enable Queue” and “Enable Spooler” and the Delete button will
become active.

Select the exams to be deleted.

To execute the command, click on “Delete” and confirm the deletion of all
selected exams.

Re-enable both buffer filling and image transmission by clicking on “Enable
