Uploading a health file using the Student Upload link

Go to gus.uams.edu and login with your UAMS/GUS credentials.

After logging in, select Student Uploads under the Student Health Information section of your GUS
Homepage

On the Student Upload screen, click Add to attach a file.

Use the file browser to select a file from your PC and then select Upload to add the file.

Once you have uploaded a file, you can view the file by selecting View

To upload additional health files, click the plus (+) sign next to the last uploaded document. This will add
a new row that can be used to attach an additional document. Continue to use the plus (+) sign as
needed until all documents have been attached.

As a final step, before you exit the Student Uploads screen, be sure to click the Save button below the
document table. This will ensure Student and Employee Health Services (SEHS) can retrieve the files you
have uploaded.

Deleting a health file from the Student Uploads document table


If you believe a document has been uploaded to the Student Uploads document table in error, please
contact a representative at Student and Employee Health Services (SEHS) to delete the file.

View and Print your Student Health Record
Go to gus.uams.edu and login with your UAMS/GUS credentials.

After logging in, select Student History/Reports under the Student Health Information section of your
GUS Homepage

The Student History/Reports page contains three tabs that provide you with information about your
Immunization Record, TB Screening and Titer Results, respectively. In addition, a fourth tab allows you
to print your Immunization, TB or Titer record.

To print your student health information, select the Immunization/TB Reports tab. In the Printable
Reports Table, select the report you wish to generate.

Once a report is selected, a secured PDF of the corresponding Student Health Record will be generated
similar to the one pictured below. The file may then be saved to a PC or printed.

Correcting Information in your Student Health Record


If at any time you believe the information contained in your Student Health Record is incorrect, please
contact Student and Employee Health Services (SEHS).
