REMARKS:
 (1) Activity Description and Inspection Item: must be described the scope of the Inspection and the Item to be inspected (ex. SONIC INTEGRITY TEST ON PILE-XXX) or the Group of the Item (ex. SONIC INTEGRITY TEST ON PILES OF AREA X)
 (2) Unit and Area
 (3) Date & Time for Inspection: If the Inspection is foreseen for a Group of Item and will be held for several days, in the request for Inspection must be indicated the start of the inspection. Ex. START 10.04.2016 h14:00 If the Inspection is foreseen for a Single Item must
indicated the date and the Time for the Inspection, for ex. 10.04.2016 h14:00
 (4) Attendance: must be reported what is foreseen in QCP (ex. H, W, W)
 (5) New RFI or Resubmitted: If the Inspection is held for the first time must be filled with NEW, vice versa with RESUBMITTED
 (6) Subcontractor Reference Inspector mobile number: Name and Phone number are useful in order to coordinate the meeting on Site for the Inspection. The Person in charge must be the same that Sign RFI according to project procedure
 (7) QCP Code and QCP Step: ex. QCP-1400 E.1
 (8) QCF Code or Inspection Report Code attached: Must be attached the QCF, Inspection report or Subcontractor Form already prepared (if is possible) that is foreseen in QCP for the acceptance of the Inspection. In case of Report that will be issued after Inspection must be
marked WILL FOLLOW
 (9) Reference Drawing attached: Attached Drawing must be attached if are useful for the Localization of the Inspection or for the definition of the Items if the Inspection cover a Group of Items, ex. PLOT PLAN marked Up
 (10) In case of single Item Inspection, pls indicate the relevant TAG; in case of inspection performed for a Group of Items, a list of Items including Tags must be attached to this RFI .
