    I, HARVEY YAP VIJUAN, of legal age, Filipino and resident of Brgy. Loboc, Lapuz, Iloilo City,
    Philippines, after having been duly sworn to in accordance with law, do hereby voluntarily depose and say;

    1. That I am the registered owner of SPARKS BUILDERS, located at Brgy. Loboc, Lapuz, Iloilo
    City;

    2. That we are License Contractor under the Company name of SPARKS BUILDERS since June
    08, 2023 with PCAB No.: 55164;

    3. I hereby commit to deliver CONCRETE READY MIX pursuant to the Construction of Two
    (2) Units Three (3) Classrooms Building (3CL) Of Abilay Elementary School located at
    Barangay Abilay Sur, Oton, Iloilo; and

    4. That I am executing this Affidavit to attest the truth of the foregoing statements and for whatever
    legal purpose it may serve.
