This contract/agreement is entered into by and between the NIA-IMO, Rosario, Agusan deln
Sur, represented by RODOLFO B. CANDARE, proprietor/manager herein after referred to as the
contractor;

sub-contractor;
Witnesseth that:
 WHEREAS, the contractor has been awarded by the NIA the opportunity to undertake works
for the Repair/Rehabilitation of Andanan-Wawa RIS- LMC-2013-ANDA-WAWA RIS-002-IMO-
ADS, located at Bayugan City, Agusan del Sur;

WHEREAS, the sub-contractor agrees to work and has the capacity to provide the
equipments for the construction of Main Canal with the amount of Eighty Seven Thousand Pesos (P
87,000.00) only per 20.0 lineal meter of the said canal and 65.0 % of Six Hundred Fifty Four
Thousand Eight Hundred Sixty Six Pesos and 30/100 (P 654, 866.30) the newly introduce item which
is Road Surfacing.

WHEREAS, the items of on-going and completed works are subject for inspection of the NIA
Technical Staff so as to the quality and quantity standard of the NIA:

NOW THEREFORE, premises considered the herein parties agree to enter into this contract
with the following terms and conditions:

1. To undertake the construction of the specified item of works with close

coordination of both parties and NIA Project Engineer.

2. To accomplish the contract works undertaken at the specified period.
