1.0Statement of Commitment to Comply with OSH Requirements

I, FRANCIS E. LAYUG by accomplishing this Construction Safety and Health Program do


hereby commit and bind ourselves to comply with the requirements of the Department Order No.
13, series of 1998 – Guidelines Governing Occupational Safety and Health in the Construction
Industry and the applicable provisions of the Occupational Safety and Health Standards (OSHS).
I also acknowledge our obligation and responsibilities to provide throughout the course of the
project the appropriate Personal Protective Equipment (PPE) as added protection for our workers
and employees.
I also hereby commit to implement the necessary job safety and health instructions and
training to all our workers for the entire (during the) duration of the project, as well as the safety of
the general public.

2.0 Company Safety and Health Policy


It is the general policy of D' BROTHERS INTERNATIONAL TRADING CORP.to perform work in
thesafest manner possible consistent with good construction practices. To fulfill the requirements of
this policy, an organized and effective Construction Safety and Health Program as described in this
program and in consonance with DOLE D.O. No. 13 and the OSH Standards must be carried out on
every project.
To achieve these objectives, we are committed to perform the following:
 Identifying potential hazards that may arise from the programmed work activities;
 Organizing the work activities so as to minimize the risk arising from them;
 Developing and implementing project specific safety and health program;
 Establishing and enforcing all necessary safe work procedures, rules & regulations;
 Ensuring that all workers are given orientation/briefing or induction prior to deployment to
the site;
 Establishing a site safety and health committee to act as policy making body of the project
on all issues pertaining to safety and health;
 Ensuring the conduct of daily toolbox meetings;
 Establishing a system of follow-up of actions and periodic assessment to check program
effectiveness.

Construction Safety & Health Committee


Section 11 of D.O. No. 13 requires that rules of Construction Safety and Health Program must be
observed and enforced at the project site, each site shall, at the start of the construction have a
construction safety and health committee. With respect to this project, the committee will be
organized in accordance with the requirements of Rule 1040 of the Occupational Safety and Health
(OSH) Standards of the Department of Labor and Employment (DOLE).
The health and safety committee is the planning and policy making group in all matters pertaining
to safety and health. The principal duties of the Healh and Safety Committee are:
 Plans and develops accident prevention programs for the construction project;
 Directs the accident prevention efforst of the establishment in accordance with the safety
programs and performance and government regulations in order to prevent accidents from
occurring in the workplace;
 Conducts safety meeting at least once a month;
 Review of reports of inspection, accident investigation and implementation of program;
 Submits reports to the project manager on its meeting and activities.
 Provide necessary assistance to government inspecting authorities in the proper conduct of
their activities such as the enforcement of the provisions of DOLE’s Occupational Safety and
Health Standards;
 Initiates and supervises safety training for employee; and

 Develops and maintains a disaster contingency plan and organizes such emergency service
units as may be necessary to handle disaster situations pursuant to the emergency
preparedness manual of the Office of Civil Defense.

The principal function of the Safety Man assigned to this project is to act as the employer’s (needs
identification) principal assistant and consultant in the application of programs to remove the hazard
from the workplace and to correct unsafe work practices. For this purpose, the Safety Man has the
following duties:
4.3.1 Serves as Secretary to the SHC, as such will perform the following:
o prepare minutes of meetings;
o report status of recommendations made;
o notify members of the meetings; and
o provide safety information to all employees
o resolve safety problems
o recommends safety measure to prevent recuccurence of accident
o conducts safety and health inspection as member of the commitee

6.1 Workers Safety and Health Orientation/Trainings


We require new workers to attend our Safety & Health Orientation before they are deployed to this
project site in consonance to Section 12 of the D.O. 13. The assigned Safety Officer JAETHAMIE
DAGANI will coordinate the conduct of this orientation.
We ensure that they receive instruction and training regarding the general safety and health
measures we plan to implement for this project, specifically:
 basic rights and duties of workers at the jobsite;
 means of access and egress both during normal work and in emergency situation;
 measures for good housekeeping;
 location and proper use of welfare amenities and first –aid facilities;
 proper care of PPEs and other protective clothing;
 general measures for personal hygiene and health protection;
 fire precautions to be taken;
 action to be taken in case of any emergencies;
 requirements of relevant health and safety rules and regulations.

7.0 Toolbox Meeting


D.O. No. 13 define toolbox meeting or gang meeting as a daily meeting among workers and their
respective supervisors for the purpose of instruction, discussion and proper briefing on the planned
work, the assessment of past work, the possibility or actual occurrence of accidents at the site, tips
and suggestions on how to prevent possible accidents and other related matters.

7.1 Responsible for the Toolbox Meeting


The following supervisor or any designated person (e.g. foreman, leadman, gangboss, etc.) is
required to conduct daily toolbox or similar meetings prior to starting the tasks for the day to
discuss with the workers and anticipate safety and health problems related to every task and the
potential solutions to those problems. The supervisor will remind the workers on the necessary
safety precautions that need to be undertaken.

8.0 Accident/Incident/Illnesses Investigation & Reporting


We consider accident/incident/illnesses investigation and reporting as our responsibility.
Absenteeism can greatly affect our work schedule. Looking for replacement can be costly for the
company, the hiring effort, training of new workers, the loss of job momentum affect our
productivity. We involve our supervisors in this program because they are more familiar with the
people involved, they have better understanding of the operation, and they have personal interest
in the investigation since it’s their people who are involved. For this reason, the D’BROTHERS
INTERNATIONAL TRADING CORP is committed to include this responsibility to all supervisors and
made them aware about it.
8.1 Persons responsible for conducting investigation

Personal Protective Equipment (PPE)


Section 6 of D.O. No. 13 requires that every employer shall, at his own expense, furnish his workers
with protective equipment for eyes, face, hands, feet, lifeline, safety belt/harness, protective shields
and barriers whenever necessary by reason of the hazardous work process or environment,
chemical or radiological or other mechanical irritants or hazards capable of causing injury or
impairment in the function or any part of the body through absorption, inhalation or physical agent.
Provision of PPE shall be in accordance with Rule 1080 of the OSHS (must include Rule 1070 for
noise). The equivalent cost for the provision of PPE (life span, depreciation, replacement, etc.) shall
be an integral part of the project cost.

9.1 Cleaning and Proper Maintenance of PPEs


It is important that all PPE be kept clean and properly maintained. Cleaning is particularly important
for eye and face protection where dirty or fogged lenses could impair vision. Our workers or
employees inspect, clean, and maintain their PPE according to the manufacturers’ instructions
before and after each use. Our supervisors are responsible for ensuring that users properly maintain
their PPEs in good condition.

We comply with Rule 1412.09 of the DOLE OSH standard in providing a safe covered walkway over
the sidewalk for use by pedestrians in a water supply construction work less than 2.3 meter (7ft.)
from a side walk or public road.
We comply with Rule 1412.10 of the OSH Standard, Protection from Falling Materials. We observe
the following:
We will take the steps to protect worker from slippery or hazard area, such as the provisions of
wearing the safety shoes and helmets.
We will ensure that tools, objects and materials including waste materials will not be thrown or
tipped from a height, but will be properly lowered by crane, hoist or chutes. If such is not
practicable, the area where the material is thrown will be fenced and no person will be allowed in
the fenced area.

11.0 Safety Signages


Our Safety Signages provides warning to workers and employees and the public about the hazards
around the project site. These will be posted in prominent positions at strategic location visible as
far as possible in a language understandable to most of the workers and employees as well as the
public.
11.1 The specific safety signages we intend to set-up for this project include but not limited to:
Mandatory requirement on the usage of PPE prior to entry to the project site

Areas where there are potential risks of falling objects;

Areas where explosives and flammable substances are used or stored;

Areas where there are tripping or slipping hazards;

Approaches to working areas where danger from toxic or irritant airborne contaminants/substance
may exist which should indicate the name of the contaminant/substance involved and the type of

All places where contact with or proximity to electrical/facility equipment can cause danger;

All places where workers may come in contact with dangerous moving parts of the machineries or
equipment;

Location of the fire alarms and fire-fighting equipment;

Instructions on the usage of specific construction equipment;

11.2 Inspection and Maintenance of Safety Signages


We will regularly inspect and maintain in good condition all safety signages that we will be providing

14.0 Control Measures on Construction Activities
To ensure safe and healthy working conditions throughout the duration of the project the following
control measure activities will be enforced and disseminated to all the workers in the site:
 Protect the environment and surrounding communities from hazards and environmental
concerns posed the construction activities in the project.

a. Mount equipment so that vibration will be reduced
b. Position equipment as further away from the field officers and active areas as
practicable.
c. Install muffler to exhaust systems of machines and equipment
d. Provide ear protection for workers exposed to noise

c. Implement site speed limit at 20kph or as required by the owner for all vehicles

Physical hazards are the most common in most workplaces. The physical hazards that we identified

14.2.4 Ergonomic Hazards


Ergonomic hazards occur when the type of work, body position and working conditions put strain on
the body. They are the hardest to spot since one does not immediately notice the strain on your
body or the harm these hazards pose. Short-term exposure may result in "sore muscles" on the
days following exposure, but long term exposure can result in serious musculoskeletal injuries.

14.3 Safe Work Practices


Safe work practices are procedures adopted for carrying out specific tasks that ensures workers'
exposure to hazardous situations, substances, and physical agents is controlled in a safe manner. Safe
work practices are generally written methods outlining how to perform a task with minimum risk to
people, equipment, materials, environment, and processes. It should be developed as a result of
completing a Hazard Assessment and should closely reflect the activities in this project.
All safe work practices should be kept in a location central to the work being performed and readily
available to the workforce. Some safe work practices will require specific job procedures, which
clearly set out in a chronological order each step in a process.

15.0 First-Aid, Health Care Medicines and Equipment Facilities
The company will provide first-aid kit and health care medicines and facilities for workers in the site
in accordance with the requirements of Rule 1960 of the OSHS.
First aid facilities are provided for workers safety. Qualified personnel are available to render
treatment and to maintain required records.
a. Report all inquiries immediately, no mater how mintor, to the Safety Officer and First Aider.
The treatment will be given, and the incident will be recorded. You must notify your
supervisors and First Aider prior to leaving the jobsite because of injury or illness, whether
personal or work-related.
b. If you get outside medical treatment (without clearing through the First Aider) for a work-
related injury or illness, you must notify the First Aider at the start of the next schedule
workday. Failure to do so may result in disallowance of your claim and/or discharge.
c. Prior to returning to work after a disabling injury or illness, you must present a medical
clearance from the attending physician to First Aider.

16.0 Workers Welfare Facilities


The following welfare facilities will be provided in the site to ensure human working conditions:
 Adequate supply of safe drinking water
 Adequate sanitary, washing and sleeping facilities separate for men and women workers
 Adequate facilities for changing and for the storage and drying of work clothes.
 Adequate accommodation for taking meals.
 To assure that the company provides adequate welfare facilities for the workers in the site
the company will implement the provision of toilets and other facilities in accordance with the
requirements of the Sanitation Code.

17.0 Medical Surveillance


The company will require all employees to undergo a baseline or initial medical health examination
prior to assigning to a potentially hazardous activity. The examination will include but not limited to
the following:
 Complete medical and work history;
 Physical examination (Pre-employment, During employment and Separation);
 Other special examination (Pulmonary function test, blood panel, ECG >40 years of age,
audiogram);
 Random drug testing.

Construction Waste Disposal
The company including subcontractors will be responsible for minimizing waste generated during the
implementation of the project. The following procedures for disposal of wastes will be implemented in
the site:
 Ensure that the construction wastes are segregated from that of domestic waste.
 All domestic wastes are to be collected on a daily basis.
 Construction debris (broken hollow blocks, spoiled concrete, loose concrete, etc) should be
taken out on the staging area.
 Oil spills and spoiled greases should be wrapping in the black garbage bag and will be properly
disposed.
 Application of good housekeeping.
Emergency Preparedness
The objectives of this are to ensure that the company has developed and communicated plans that will
allow for the effective management of emergencies. Attach copy of company emergency preparedness
plan.
a. Objectives
The Objectives of this Emergency Action Plan are:
1. To ensure readiness to cope with emergency that may result in the loss of life and property
2. To be able to effectively carry out an orderly, timely and safe evacuation of all personnel in
affected ares of the emergency when necessary.
3. To prevent further injuries or casualties, and more damage to properly in time of disasters, or
natural calamities.
4. To facilitate first aid response to an injured worker and immediate access to emergency room

b. Emergency Procedure
Should emergency occur, the Project In-charge on site shall:
1. Immediately secure the area and implement the emergency action plan. According to the
situation, the person-in-charge will delegate authority and assure the safety of those
responding.
2. Ensure notification of the project safety officer.
3. Provide information regarding the emergency to rescue personnel and project management
representatives only.

Job Hazard Analysis


a. Objectives
The objective of hazard analysis is to develop a preliminary hazard analysis on work operations
asto their potential for injury, property damage or both. Once potential hazards have been
identified, procedurescan be developed to eliminate or mitigate their potential for occurrence.
A toolbox meeting will be conducted every morning for 15 minutes, here they will identified
hazard in every scope of work and give corrective action in order to avoid accidents.

c. Standard Operating Procedure


Site Clearing
o Before implementation of the project, the area must totally cleared from all danger or
harm to avoid damages from materials, equipment to manpower.
o The operations plan is prepared before any earth is moved. The plan includes the limits
of the site to be developed, methods of excavation, equipment to be used, number of
personnel required, and locations of roads, structures, and support equipment.

General Trenching and Excavation Rules


 Keep heavy equipment away from trench edges.
 Keep surcharge loads at least 2 feet (0.6 meters) from trench edges.
 Know where underground utilities are located.
 Test for low oxygen, hazardous fumes and toxic gases.
 Inspect trenches at the start of each shift.
 Inspect trenches following a rainstorm.
 Do not work under raised loads.

The precautions to be taken are:

1. Trench collapse should be avoided by battering the sides to a safe angle or by supporting them
with sheeting or proprietary support systems. Support should be installed without delay as the
work progresses. Ensure the workers are competent and experienced as far as possible and that
they have clear instructions.
2. Excavated spoil, plant or materials should not be stored close to the sides of excavations as
loose material can fall in. The extra loading can make the sides of the excavation more likely to
collapse.
3. Prevent people from falling into excavations by substantial barriers around the edges. This must
be done if the depth exceeds 2 metres but is recommended for excavations of lesser depths.
4. Prevent vehicles from falling into excavations or surcharging and causing the collapse of the
sides of the excavation by keeping them out of the area. Baulks and barriers can be provided
for this purpose and should be painted to be easily visible. If vehicles have to tip materials into
excavations then they should be prevented from over-running into the excavation by using stop
blocks.
5. Provide safe access in and out of the excavation.

Handling hazardous and/or toxic chemical substances


o Whenever possible, it is always best to avoid using a toxic material either by eliminating
its use (by changing the method or process for example) or by substituting the toxic
material with a less hazardous material. Unfortunately, it is not always possible to find a
non-toxic substitute that still does the job effectively and safely.
o When considering substitution, the first step is to obtain the Material Safety Data Sheets
(MSDSs) for all possible substitute materials. Find out about all of the hazards (health,
fire, chemical reactivity) of these materials before making any changes. Caution must be
exercised so as to avoid introducing a potentially more hazardous situation. Choose the
least hazardous materials that can do the job effectively and safely. Learn how to work
safely with them, too.

 Temporary Electrical Connections and Installations


The major hazards of electricity are electrical shock and the accompanying possibility of death.
Fatal electrical shock can be sustanined with voltage of 120 or less with the current as low as
50-200 mill amperes. The major soruce and distribution equipment (generators, transforemrs
and switch racks) shall be isolated or in building with access restricted to authorized personnel
only.
o All temporary circuits at the construction site shall be grounded with the ground circuit
having a resitance no greater than 25 ohms.
o All circuits shall be provided with fuses or circuit breakers sized to prevent overloading of
the wire and equipment as the circult.
o All non-current carrying metal frames or containers of electrical equipment shall be
grounded.

A tag and lock procedure out procedure shall be established and used for any work to be
done on circuits, or equipment powered by energized circuits. When working in an operating
substation or substations with energized buses, barriers shall be provided and identification
signs installed to segregate workers from “live” equipment. All portable electric powered tools
shall be grounded or double insulated. Cords for these tools shall be run and located to
minimize possible damage to cord. Cords shall not be used for hoisting tools. Where more than
one voltage level is in service at a site, each receptacle shall have its voltage identified.
 Use of power tools and equipment
Appropriate personal protective equipment such as safety goggles and gloves must be worn to
protect against hazards that may be encountered while using hand tools.

Workplace floors shall be kept as clean and dry as possible to prevent accidental slips with or
around dangerous hand tools.

Power tools must be fitted with guards and safety switches; they are extremely hazardous when
used improperly. The types of power tools are determined by their power source: electric,
pneumatic, liquid fuel, hydraulic, and powder-actuated.

 Gas and electric welding and cutting operations


o To prevent injury to personnel, extreme caution should be exercised when using any
types of welding equipment. Injury can result from fire, explosions, electric shock, or
harmful agents. Both the general and specific safety precautions listed below must be
strictly observed by workers who weld or cut metals.
o Do not permit unauthorized persons to use welding or cutting equipment.
o Do not weld in a building with wooden floors, unless the floors are protected from hot
metal by means of fire resistant fabric, sand, or other fireproof material. Be sure that hot
sparks or hot metal will not fall on the operator or on any welding equipment
components.
o Remove all flammable material, such as cotton, oil, gasoline, etc., from the vicinity of
welding.

Company Policy on Drug-Free Workplace
In compliance with Article V of Republic Act No. 9165, otherwise known as the Comprehensive
Dangerous Drugs Act of 2002, and its Implementing Rules and Regulations and DOLE Department
Order No. 53-03, series of 2003 (Guidelines for the Implementation of a Drug-Free Workplace Policies
and Programs for the Private Sector D’BROTHERS INTERNATIONAL TRADING CORP hereby adopts the
following policies and programs to achieve a drug-free workplace:

while performing an assignment.
 Being impaired or under the influence of dangerous drugs away from the company, if such
impairment or influence adversely affects the employee's work performance, the safety of
the employee or of others, or puts at risk the company's reputation.
 Possession, use, solicitation for, or sale of dangerous drugs away from the company
premises, if such activity or involvement adversely affects the employee's work
performance, the safety of the employee or of others, or puts at risk the company's
reputation.
 The presence of any detectable amount of dangerous drugs in the employee's system while
at work, while on the premises of the company, or while on company business. "Dangerous
Drugs" include those listed in the Schedules annexed to the 1961 Single Convention on

Narcotic Drugs, as amended by the 1972 Protocol, and in the Schedules annexed to the
1971 Single Convention on Psychotropic Substances as enumerated in the attached annex
of R.A. 9165.
 Treatment, Rehabilitation, And Referral
o An officer/employee who, for the first time, is found positive of drug use, shall be
referred for treatment and/or rehabilitation in a DOH accredited center. For this
purpose, D’BROTHERS INTERNATIONAL TRADING CORP shall provide a list of at least
three (3) accredited facilities which an employee who was tested positive for drugs
may choose from.
o Repeated drug use even after ample opportunity for treatment and rehabilitation
shall be dealt with the corresponding penalties under R.A. 9165 and is a ground for
dismissal.
