• Automatically created after reconstruction problems
• Create manually: GetBirInfo.csh
• Located in /toshiba/system_data/log/syslog/
• Open errlog.BIRXXXXXXXX.txt e.g. with jot editor
• Summary at the top
• Check for errors under “Errors found”
• If a slot-number is listed - replace board in that slot

MHR Diagnostics FSM 3120
• Connect network cable between RTM and HUB - straight -
• Connect serial RS232 cable between RTM and PC - 0 modem -
• Start Teraterm on the PC
• Switch on the system - including gantry or last FC will fail
• Press any key (not [shift], [ctrl] etc.) to the message
- press any key for service menu -
• Configure the RTM in the network - Modify BootLine -
• Reboot the system - abort RTM boot-up - allow the scan console to boot
• Type 1[Enter] to select - run diagnostics -
• Type n[Enter] to the message
- Failed to connect to aqm as vxworks do you want to retry? -
• Diagnostic menu is displayed

• Test requires serial number on each MHR board
• Serial number on bar code label and in flash ROM - done at the factory
• No need to change - unless not written or conflict with other boards

To write serial number


- set JP2 and JP3 to 1-2
- Start MHR diagnostics and enter the number
- set jumpers to original position

Situation
Root partition was 100% full

Cause
“Core” files are created in /var/adm/crash- in this case with DIMM problems

Removed “core” files in /var/adm/crash

FC driver
Install the FC driver manually if it is not installed automatically

1. Backup Rawdata and Image data and Calibration data, if necessary


2. Open winterm
3. change to super user: % su [enter], password "Aquilion"
5. Type: chkconfig aquilion off
6. shutdown the system
7. reboot the system
8. open console
9. change to super user
% su [enter]
10. type password "Aquilion"
11. move to /ct/KBProj/develop/etc/scan/BIR/fcdriver.
% cd /ct/KBProj/develop/etc/scan/BIR/fcdriver [enter]
12. type command described below.
# ./INSTALL [enter]
13. Do RDDsetup

RAID level 3 - 1/3
After reloading V1.40ER003, reloading RAID configuration file from MOD
(site specifics backup) does not work. An error message is generated that
the RAID configuration file is missing. Restoring the file from RDD with the
RDD setup menu took care of the problem.

• Remove disk 122
• Reboot - three error messages
• Click 3x OK and the system runs fine
“non-redundant mode”
• After reinserting the disk not recognized as data disk
recognized on the fiber channel
only two error messages remain

• RDD setup Add disk to RDD progress window hangs at about 37%
RTM terminal hangs at about 66%
LED’s on the RAID continue to blink
Connect serial cable to another PC - progress counter is updated again
Only the counter display hangs up

• After a while the progress window shows “done” and a message is shown
that the procedure ran to completion

Avoid this problem by correct system selection!
Wrong system selection 1/2
• Double-check system selection before SET
• Wrong selection - Problem!
• No boot-up if Aquilion/16 is set for D-console instead of Aquilion/16(Sub)
Error message - failed to mount
Tries to mount dev/dsk/dks0d2s2 to /ct and dev/dsk/dks0d2s3 to /recon
• Switching off the aquilion flag “chkconfig aquilion off” does not help: it still
fails to boot to the configuration menu

Remedy
• Edit /etc/fstab to set correct mount point for /ct: disk dev/dsk/dks0d1s2 disk
1 for the Octane2, instead of 2, as on O2 R12K; # out the other partitions
• From single user check if directory /ct is mounted by typing mount and
then cd /ct and remove any files that have been put into this directory. If it
is not mounted it should be empty - rm -r "filename"

Avoid this problem by correct system selection!
Wrong system selection 2/2
• Boot to system setup menu and make the correct scanner selection
• Reboot - no need to reload software!

Notes
To edit /etc/fstab, go to single user and use vi. Or, when the system errors out it will
drop into the unix desktop after a while. From tool chest select unix shell from the
menu desktop and use jot to edit /etc/fstab

/etc/fstab is a table of file systems to mount to devices

This procedure was successful in the event where on the D-console the selection
for the S-console was made. It may also work if the wrong selection is made on the
S-console. Note the correct disk number on the scan side though. The D-console
system disk is dks0d1 and the S-console system disk is dks0d2
