1. GENERAL
1.1 Purpose of the Document
The purpose of this document is to describe the procedure implemented for the container house material
unloading operations to take place at MCPY 6, MCPY 8 and MCPY 10. The unloading operation will
involve the use of a crane and will be carried out in accordance with relevant safety regulations and
guidelines.
1.2 Scope
This method statement covers all aspects of the lifting operation. This will involve the loading and
unloading process of the prefabricated houses involving a mobile crane.

Confirming with the Crane/Forklift Truck Operator that the lift is within the crane’s safe operating
radius regarding LOP.
Ensuring that all master links, slings, and shackles are secured, located correctly, and have
freedomof movement.
Ensuring that, where appropriate, taglines are fitted and controlled by the Slingers.
Ensure that crane hook is positioned above centre of gravity before sending any signal to the
Crane Operator.
Placing themselves in a prominent position where he/she can be safe and clearly seen and safely
observe the entire operation.
Remaining “hands-off” the load during the lifting of any loads, although training / guidance on
slinging practices is permitted prior to the operation.
Adopting standard crane signals unless other methods of signalling are to be utilised, such as
two-way radios.
Clearly indicate to the crane operator where the load must be moved to or placed and, where
possible, follow each load to its destination.
Participating in toolbox talks, job completion reviews and return of experience.
Participate in the risk assessment for non-critical and critical lifting operations.
Ensure all the works mentioned in this procedure are carried out safely with minimum risk to the
workers, equipment and materials.
Monitor the implementation of H3SE procedures at work sites.
Ensure all relevant safety regulations of company and contractor are observed.
Ensure all employees are given instruction and understand on their working responsibilities.
Carry out inspection and issue reports on unsafe conditions/actions.
Be responsible for other works related with H3SE at site.
Disclosure the requirements of H3SE.
Discipline, re-educate and/or train any member of staff failing to discharge satisfactorily their
responsibilities for Health, Safety and Environment.
Arrange any necessary special training for all (E.g. H3SE, defensive operating training, etc.),
particularly expatriate operator.
Ensure that all personnel are in possession of the appropriate licenses or permits and any
specific training, that they are aware of any special requirements and restrictions that may be
applicable.
Ensure all accidents are reported as required by contractor policy, accident reporting and
investigation as required by the document UT-MID-60-CPP1-110015 Incident Reporting and
Investigation Procedure;
Read and understand the COMPANY and contractor safety policy and carry out work in
accordance with relevant requirements.

Knows the rigging and mechanical - handling operations techniques (incl. cross-hauling, etc.) and
how to use/operate lifting equipment (winch, chain and lever hoist, cable, etc.) in limited/confined
environment.
Collect the weather conditions of the day, report to the lifting commander, and determine the
lifting start time.
Coordinate with cable workers, cooperate with hoisting inspection, and direct the movement of
equipment transport vehicles.
Visual inspection of lifting equipment/accessories before using.
During the lifting process, pay attention to the information feedback of other personnel,
immediately stop the operation if any abnormality is found, and can be lifted again after returning
to normal.
Pre/Post-Use inspection of lifting equipment.
Ensuring the correct selection (accordance to LOP) and fitting of appropriate rigging and
hardware with due consideration given to the protection of the load.
6.5 Operator
Should be familiar with the vehicle performance parameters and understand the maintenance
knowledge.
Should be familiar with vehicle operation methods, familiar with lifting operation process.
Understand the site situation, the station position situation, stop and operate according to the
designated position.
Should be familiar with command signals, gestures, flags, whistles, familiar with radio use
methods.
Observe the command signal, and any signal received must be strictly executed.
If any abnormality is found, timely feedback to the heavy industry, and continue the operation
after returning to normal.
Ensuring the lifting equipment is roadworthy (for mobile crane), functioning correctly and is
properly maintained every time that the item of lifting equipment is being operated.
Establishing the weight of the load prior to starting any lifting operation. No load is to be lifted
where the weight is unknown.
Establishing which signalling system is to be used and following instructions from only one
signaller (Signalman /Banksman), except the emergency stop sign which can be given by any
individual.
6.6 Spotter/Flagman
Traffic control: responsible for controlling traffic and ensuring that pedestrians stay clear of the
crane's operation zone to prevent accidents.
Clearance and obstacles control: keep an eye on the surroundings to prevent the load from
coming into contact with obstacles or structures.
Safety compliance: ensures that all safety regulations and guidelines are followed during lifting
operations.

6.7 Coordinator
The coordinator is responsible for the entry and exit of the field vehicles, the entry of relevant
personnel, the preparation, distribution and recovery of the on-site articles and tools, the
coordination and organization of various departments, and the arrangement and organization of
conference training. Construction coordination of site related parties, related resource allocation,
etc.
In lifting operations, every task can vary, and for safety purposes, it is essential to conduct a 'risk
assessment for each one. The extent of this risk evaluation will expand as the task's complexity grows,
and there are a multitude of factors to consider during the planning phase
The weight of the load.
Position / height of the centre of gravity.
Stability of the load.
Size and shape of the load.
Potential fragility of the load.
Availability of dedicated lifting points on the load.
Availability of suitable rigging.
Protection of rigging against sharp edges.
Capacity of the crane / hoisting equipment.
Availability of certified anchor points / support steelwork.
Available headroom.
Maximum height the load has to be lifted.
Any dynamic factors

Experience / competence of personnel
The occurrence of site accidents should be kept to a minimal. JSA Document shall be approved by
COMPANY to cover the step risks and mitigation recommended before the work start. Health, Safety and
Environment Plan will be provided at site to ensure smooth running of works in safe manner. This is a
technical construction procedure, herein, it is a brief overview.
Before the lifting, organize all the participating lifting personnel to conduct technical training,
clarify the lifting process of equipment lifting, and ensure that the lifting personnel can proceed
smoothly according to the lifting procedures.

Organize all the participating lifting personnel to make technical disclosure before the lifting. The
disclosure content should be comprehensive, profound, and detailed, so that the lifting personnel
can clarify the lifting procedures of each equipment to ensure the lifting quality.
The hoisting command shall be composed of personnel with practical experience, high technical
level and strong organizational ability. The hoisting command must fully understand the hoisting
scheme, timely carry out the post division of labour, and well disclose the post responsibilities and
lifting operation technology.
The hoisting command must strictly implement the hoisting scheme, and solve the problems
found with the scheme preparation staff in time.
In the comprehensive inspection process and test lifting process of lifting, all post personnel shall
check, identify, and maintain with strict operation procedures.
During the lifting process, all personnel should report the problems found to the lifting command
in time and continue the lifting operation can be completed after the problems are finished.
During the hoisting process, there shall be a unified command signal. All personnel involved in
the hoisting must be familiar with this signal and obey the command.
A police cordon shall be set up in the lifting operation area and make obvious marks. When lifting
the work piece, irrelevant personnel are strictly prohibited to enter or pass through.
Lifting operators shall be familiar with the lifting plan, lifting equipment performance, operation
procedures, command signal and safety requirements, and must hold the operation certificate of
special operators.
Lifting units before lifting personnel safety technical disclosure, operation must be unified
command, participate in the hoisting lifting personnel must keep post, work according to the
command, the hoisting process, any post failure, must immediately report to the commander, no
command, no one shall command or evacuate post without authorization.
The lifting plan or lifting technical measures shall be prepared for the hoisting of the equipment or
components and submitted to the relevant leading departments for approval according to its
importance. Without the permission of the supervisor, the lifting plan shall not be changed
It is strictly prohibited to use lifting equipment with overload. The lifting should be stable to avoid
vibration or swinging. Remove the sling or unhook before the sling is fixed.
The sling shall be tied to the centre of gravity of the heavy object and tied symmetrically and
firmly to prevent heavy dumping or slipping. If the sling must be tied below the centre of gravity
due to special circumstances, effective safety measures shall be taken and approved by the
general technical responsible person.
If the weight surface has angular edges, effective protective measures should be taken when
tying the sling. Before lifting operation, a firm rope should be attached to the heavy object.
Prevent heavy objects from swinging, rotating, or colliding with other buildings and structures
during the lifting process.
No person shall lift or lifting machinery. In special circumstances, safe and reliable measures
shall be taken and approved by the relevant safety department.
Before the formal lifting of the equipment, test lifting should be carried out. Before the test lifting,
the lifting personnel should strictly check the installation of the lifting ear, machinery and rigging
according to the division of labour, confirm that it meets the requirements of the scheme, and
then carry out test lifting. If problems are found, they should be done first.
The piece is put back to the ground, and the crane is tested again after troubleshooting, to
confirm that everything is normal before formal lifting.

Before lifting the equipment, contact the local meteorological department in advance. When the
wind speed reaches 9 m / s, outdoor lifting operation shall not be carried out.
The lifting weight shall not stay in the air for a long time. If a stay is necessary, reliable measures
must be taken. Within the scope of lifting operation, personnel are strictly prohibited to pass
through. During the course of the operation, no one shall stand vertically below the lifted load.
7.3 Emergency
If any unexpected incidents occur during site take over, as personal injury or traffic accident. The
preliminary emergency procedure shall be started up quickly.
The Site HSE department shall arrange and coordinate with site execution team in arranging
emergency procedures.
Site first aid room is established to carry on first aid on site in case of any injuries.
In case of any emergency the Site in charge will be informed about the emergency.
Emergency assembly point shall be allotted and communicated to all the operatives involved in
the job.
Clear access to the emergency assembly area shall be provided with signage in place by safety
officers.
No construction activity shall block the access leading to the emergency assembly point. In case
of any deviation the access has to divert and the same shall be communicated to all the
operatives on Site.
8. EXECUTION
8.1 Lifting preparation
Site conditions
- Ground condition; Ensure that the ground where the crane will be positioned and where the load
will be placed is stable and capable of supporting the crane's weight, the load, and any outriggers
or support structures. Soft or uneven ground can be a safety hazard.
- Accessibility; ensure the area is easily accessible for the crane and transportation trucks making
sure there is enough space for manoeuvring
- Environmental condition; Consider weather conditions like wind, rain, and lightning. High winds,
in particular, can affect crane stability and safety.

Prefabricated houses are in 2 types of boxes. One box with dimensions of 6055*2990*2896mm will have
2 houses inside with a total weight of 4114kg. Another box with dimension of 3000*2990*2896mm will
have one house inside with a total weight of 1100kg. Before the vehicles left the port, the Coordinator
from CPP contacted the EACOP Transport and Material Control Representative to determine the address
of the camp that would receive the prefabricated houses, the route of the vehicles, the number of vehicles
and the expected time of arrival. Upon departure of the vehicles, timely communication is maintained with
EACOP Transport department to Confirm accurate information on the arrival time of each truck at the
camp by checking through the IVMS system.
Before the truck arrives, all relevant operating personnel and equipment arrive at the site to prepare in
advance. Lifting supervisor summoned All personnel on site held a toolbox talk given and prep everyone
for the operations for the day. H3SE Inspector once again raised the risks of hoisting and the need to pay
attention to hoisting and check the status of the lifting equipment again before lifting, again check the PPE
worn by everyone on site. After the truck arrives, the coordinator will arrange for it to park in a unified
parking area and wait for unloading.
8.3 Setting up crane
Crane inspection; prior setting up of the crane, inspection will be done to ensure its in proper
working conditions
Outrigger placement; extend and position them properly to provide balance and stability. Use
crane pads if needed to distribute the weight evenly.
Safety measures; The lifting area will be blocked by warning tapes and all unrelated personnel
are prohibited from entering.
Boom configuration; Set up the crane's boom according to the lift plan and load requirements,
ensuring that the crane is properly configured for the task
The lifting tools are assembled and prepared
Truck inspection; to ensure it is in good working condition by inspecting the brakes, light and
mirrors
Clear communication; this is between driver and spotter to enable safe offloading
Positioning; slow and steady backup of the truck using guidance of the spotter, positioning the
truck at the rear of the crane.

9. DOCUMENTATION
All documentation associated with the lifting operations that is generated during the project must
be retained in an orderly manner within a referenced filing system.
All documentation will be kept in the project offices during implementation of MCPY and its
access road. All project lifting appliances will be thoroughly examined and certified prior to arrival
on site.
Records for all lifting equipment examinations every 3 months as part of the testing certification
process should be kept. The copies of these records will be retained in the project office.
Records of conducted monthly visual inspections on all lifting equipment and accessories should
be kept.
