IP Allocation Request Form

1. Requested number of usable IPs

EX: If you are going to be assigning 10 IPs to different systems, please enter 10.
If you are going to assign 5 IPs to the same system and an additional 6 IPs to 6
other systems, please enter 11.
If you are going to assign 4 IPs to systems immediately, and plan on assigning
an additional 20 within the next 6 months, please enter 24.

4. Please describe why you would not be able to use NAT (Network Address Trans-
lation) or it’s variants, or virtual hosting technologies (web and other) to meet your
current and future IP allocation needs.

As a member of ARIN, all ISPs must follow ARIN guidelines and policies with respect
IP address allocation. ARIN’s policy on name-based web-hosting states:
"When an ISP submits a request for IP address space, ARIN will not
accept IP-based webhosting as justification for an allocation, unless an
exception is warranted. Along with the request, organizations must pro-
vide appropriate details demonstrating their virtual webhosting cus-
tomer base"
