CLASSROOM OBSERVATION LESSON PLAN

I. OBJECTIVES
A. Content Standard The learner demonstrates understanding of key concepts of variation and radicals.
B. Performance Standard The learner is able to investigate thoroughly mathematical relationships in various
situations, formulate real-life problems involving quadratic equations, inequalities,
and functions, and rational algebraic equations and solve them using a variety of
strategies.
C. Learning At the end of the lesson, the students are expected to:
Competencies/ o Recall the quadratic equations in standard form;
Objectives o Recall on how to factor a perfect square trinomial; and
(Code) o Transforms the quadratic function defined by y=ax 2+ bx+ c into the form
2
y=a(x−h) +k .
II. SUBJECT MATTER
A. Topic TRANSFORMING QUADRATIC FUNCTION INTO THE FORM
2
y=a(x−h) +k
B. Reference Mathematics CapSLET 9, Quarter 1 Week 7 pages 1-5
C. Material Visual aids (PPT), Activity Sheets, Calculator
III. PROCEDURES
A. Preliminaries  Prayer
 Checking the student’s attendance and cleanliness of the classroom.
 Remind the students on the safety protocols inside the classroom.
B. Recall Before we start with our lesson for today, let’s have a recap on the lessons we have
discussed during our first lesson on quadratic equations and solving quadratic
equations by completing the square. We will have a two-part activity. First, you will
be grouped by your respective tables where you are seated. Next, I will be flashing on
the screen the different quadratic equations, and you will determine the values of a, b,
and c by writing it on the paper provided to you. After you have written the values,
raise your papers to see if you got the correct answer. 3 points will be given to the
group with the correct answer. Number your member according to how many you are
on your respective table. Which team with the highest points will gain 20 points for
the recitation. Let’s begin.

C. Teaching/Modeling Let us explore the two forms of Quadratic Function

What is the general form of quadratic function?
General Form
The general form of quadratic function is f ( x)=ax 2+ bx+ c which is also the
same as y=ax 2+ bx+ c.

What is the other form of the quadratic function?

Vertex Form
The vertex form of a quadratic function is given by y=a( x−h)2 +k , where
(h , k ) is the vertex of the parabola.

How do you transform general form y=ax 2+ bx+ c into vertex form
2
y=a(x−h) +k ?
Why do we need to transform general form of quadratic function into vertex
form?

Let’s start!
Here are the amazing steps in transforming the general form into vertex form of
quadratic function.
Let’s see how it transforms. Follow the footsteps!

Important note!
−b 4 ac−b
2
The vertex is at point (h, k) where h= and k =
2a 4a

( )
2
−b 4 ac−b
Therefore, the vertex is at point , .
2a 4a

Illustrative Example 1:
Transform y=x 2−4 x−10 in the form y=a( x−h)2 +k and then identify the vertex
of the given quadratic function.

Illustrative Example 2:
Transform y=2 x 2 +8 x−12 in the form y=a(x−h)2 +k by applying the formula
of h and k.

Why do you need to carefully follow the steps in transforming the general form of
quadratic function into vertex form?
How can this concept on transforming general form of quadratic function into vertex
form be applicable to real life situation?
D. Guided Practice In this activity, you will apply the steps in transforming quadratic function
2 2
y=ax + bx+ c into vertex form y=a(x−h) +k . These steps will lead you in
determining the values of h and k, wherein (h, k) are the coordinates of the vertex of
quadratic function.
ACTIVITY 2
TRANSFORMATION: THE HOWS OF US
Directions: Transform the given quadratic function into the form y=a( x−h)2 +k
by following the steps below. “MY TURN” column is done for you. Now, it is
“YOUR TURN” to fill your column with correct answers.
ACTIVITY 3
THE FORMULA TO TRANSFORM
Directions: using the formula of h and k, transform the general form y=ax 2+ bx+ c
by completing the steps from the base of the staircase going to the top to form
2
y=a( x−h) +k . Some steps are already done for you.
E. Independent Practice DECODING VERTEX
Directions: Determine the values of h and k of the quadratic function given the
vertex form and general form. Decode the message in the scroll code by writing the
letter that corresponds to the vertex of the given quadratic function. Look for the
ordered pair (h, k) in the code matrix.
IV. EVALUATION
Directions: Transform the following quadratic functions that is written in standard
form into vertex form.
1. y=−2 x 2−8 x−9
2. y=x 2 + 4 x+7
3. y=− x2−2 x−12
4. y=x 2−2 x +5
5. y=2 x 2 +28 x+142
