Dear Sir/Madam,

I am directed to notify you that the public health nursing application indexing portal is open
from Monday, February 12 through Tuesday, February 21, 2024.

1. Application requirements:

• A total of five (5) credits (English, Biology, Physics, Chemistry, and


Mathematics) in the O' level examination – WAEC, NECO, or NABTEB – in
no more than two (2) sittings.
• While a combination of WAEC and NECO is acceptable, NABTEB cannot be
combined with any of the two examinations.
• Only original O'level certificates or WAEC/NECO/NABTEB website printouts
should be attached.
• All NECO/WAEC/NABTEB results to be attached should have been verified
by the school.
• Scratch card information that can be verified online.
• A valid birth certificate from the National Population Commission or a sworn
age declaration that matches the date of birth on the practice license or indexing
application.
• Photographs for passports must be in colour, full face, in school uniform, on a
white background, and without eyeglasses.
• Marriage certificate, and newspaper publication for married students.
• Original documents, not photocopies, are expected to be scanned and attached.
Any application that includes a scanned photocopy of a document will be
rejected.

2. Indexing fee of ₦13,125 per student, including consultancy fees, must be generated on
the portal, and paid as a single payment per applicant via payment channels as specified
online on the portal.
3. Please note that the fee does not include bank fees, which vary depending on the
payment method (internet banking, ATM, or bank teller).
4. Please note that paying Indexing fees in bulk via Remita is no longer an option. All
indexing fees must be paid by the applicant (after a successful indexing application
and clearance from the school).

5. Following successful online application and payment, a cover letter containing the
names and application numbers of the students who applied for indexing should be sent
to the Registrar's office in Abuja as soon as possible.

6. A penalty of two hundred and eighteen thousand, seven hundred and fifty naira
(₦218,750) per school for late application and payment will be imposed on defaulters.

Kindly adhere strictly to the schools’ admission quota and study each application carefully
before approval.

Note: You must pass this information on to your students and carefully guide them to ensure
that they initiate the correct application on the portal, particularly when it comes to selecting
the correct nursing/midwifery specialty. Your cooperation is essential to achieving this, please.

If you have any concerns about your student's indexing application or need clarification, simply
notify us via [email protected] and [email protected]. It is recommended that you
always send your complaints via the provided email addresses and limit the number of phone
calls as much as possible. For indexing inquiries, please call any of these numbers
(08028642018, 08150837366) from Monday to Friday (10 a.m. to 4 p.m.).
