Thank you for allowing our students to immerse in your establishment. Indeed, you play a vital
role in the molding of him/her as a future ___________________________________, especially since you
have taken time to supervise him/her in the area. Kindly rate him/her in each of the traits indicated
below by checking the box which corresponds to how you see his/her performance. Traits that have not
been observed during the student’s stay may be marked NA (Not Applicable). This is for the purposes of
grading as well as for identification of the student’s strengths and weaknesses, thus objectivity is
important.
The following are the equivalent rating per number:
1 – Very Poor 4 – Good
2 – Poor 5 – Very Good
3 – Fair
