Annotation
For Objective 9, I got a rating of 5 for the Quality because I have
attached lesson plans that I made and performed during LAC
sessions as attested by our School LAC Coordinator. On the first
quarter I was able to prove that I have guided my colleagues in
planning to achieve this objective through the attached Performance
Monitoring and Coaching Form that we did during the First quarter.
I also got a rating of 5 for the TIMELINESS, because I was able to
attach MOVs across 4 quarters.

Annotation
Here is a copy of the Performance Monitoring and Coaching Form that we
conducted during the first quarter to plan for the strengthening the
relationships with parents/ guardians and the wider school community so
that we would be able to maximize their involvement in the educative process. With
this, teachers were asked to submit sample outputs of their communication with the
said stakeholders.

For Objective 15, which is the PLUS FACTOR, I got a rating of
5 for the Quality because I have attached MOV that shows that I
have performed related work/activity beyond the school / CLC.
I also got a rating of 5 for the EFFICIENCY, because I was able
to attach MOVs that details the achieved positive contribution of
my performed related work/activity to the teaching-learning
process. And another 5 points for the TIMELINESS because all
my submitted MOVs were distributed across 4 quarters.
