Use your ACLS cards/worksheet to work through the following scenarios. Submission is
REQUIRED prior to admission to your SIMULATION on-campus. Handwritten paper copy or
uploaded e-ink forms are accepted.

1. C.B. is a 70-year-old male admitted to the ED for shortness of breath and palpitations. You notice
the following rhythm on the cardiac monitor.

a. Which medication will you expect to administer to this patient? How much in mg? If 1st dose
ineffective, how much mg for the 2nd dose?

b. What therapy do you expect to do if this medication is ineffective?

c. If patient were stable/ asymptomatic, with the same cardiac rhythm, what other measures or
treatments are available?

2. N.R is a 45-year-old female admitted to the ED in a lethargic and confused state, blood pressure
86/54. Patient’s family reports that N.R. has a history of “meth use”. Blood work, IV access and
cardiac monitoring were initiated.

a. What is her cardiac rhythm?

b. What should be done immediately after recognition of cardiac rhythm with a pulse and
assessing an unstable/symptomatic patient?

3. N.R. now has become unresponsive without a pulse & respiration. CPR was initiated and first dose
of Epinephrine administered and below is his cardiac rhythm. 1st shock was given and CPR
continued for 2 minutes.

a. Upon pulse and rhythm check, the same rhythm appears, with no pulse. What is his cardiac rhythm?

b. What should be done immediately upon recognition of this cardiac rhythm without a pulse?

c. After giving the second shock, what medication and how many mg should be administered during
CPR?

4. T.O. is a 50-year-old male in the ED found to be unresponsive, without a pulse and respiration. CPR
was initiated & below is his cardiac rhythm. He remains unresponsive. This is his cardiac rhythm
below.

a. What is his cardiac rhythm?

b. Complete this diagram:

START CPR: Give O2 & Attach monitor & defibrillator à No pulse/ VF or pVT

à_________à CPR 2min (IV or IO Access) à ✔ Rhythm and ✔ pulse à No

pulse with VF or pVT à SHOCK (Defib) à CPR 2 min + __________________1mg à

_____________300mg à ✔ Rhythm and ✔ pulse à No pulse/ VF or pVT à
Resume CPR, Evaluate any reversible causes, Expert consultation.

c. What are the reversible causes? List the H’s and T’s.

5. B.B. is a 78-year-old female who arrived to the ED hypotensive, edematous & confused. Blood
work, IV access and cardiac monitoring were initiated. You notice the following rhythm on the
monitor.

a. What is her cardiac rate and rhythm?

b. List the clinical manifestations of a “symptomatic patient”:

c. Which medication will you expect to administer to this patient? How much in mg?

d. What other therapies to consider if this medication is ineffective?
