Critical Findings and Findings: Target Date of Correction on Below Finding/s: 06/08/2024
The lift was inspected on Thorough Examination and Test based on the Reference Standard at the time where the lift was installed. Listed below are the findings at the
time of inspection that should be Provided, Repair and Rectify before the given respective target date.
Minor Findings
- It is recommended that before doing any maintenance work on the equipment read and understand the following instruction given by the manufacturer to avoid any
accident or injuries and maintenance check list should be fill at all times. (7.2.1)
- Automatic electrical means for landing the lift incase of power supply failure was found not working due to weak battery supply, subjected for maintenance rectification.

- A new requirement as per the latest international standard such as protection against unintended car movement shall be provided. (5.6.7),
- A new requirement as per the latest international standard such as ascending car overspeed protection means shall be provided. (5.6.7),
- A permanently installed inspection control station operable from the refuge space in the pit area should be provided. (5.2.1.5.1)
- A sign in the pit readable from the entrance(s) shall clearly indicate the allowed number of persons and the type of posture considered for the refuge spaces(s)
accommodation. (5.2.5.8)
This certificate is granted subject to the condition that it is understood and agreed that nothing herein contained shall be deemed to relieve any designer, manufacturer, seller, supplier, repairer, or operator
of any warranty, express or implied, and ITAC Safety liability shall be limited to the acts or omissions of its employees, agents, and subcontractors. Under no circumstances whatsoever shall ITAC Safety be
liable for any injury or damage to any person or property or occurring by reason of negligent operation or any defect in all design, materials, machinery, equipment, or other items other than such defects
ascertainable by normally accepted standards and only those items actually inspected by ITAC Safety, and which covered by this certificate.
LIABILITY – The Client or his assignee of supervisor in interest agrees to indemnify and hold harmless ITAC Safety from and against any and all claims, demands, lawsuits or actions or damages, including legal
fees to persons and/or property tangible, intangible or otherwise which may be brought against ITAC Safety incidental to, arising out of or in connection with the work to be done, services to be performed or
material to be furnished hereunder except for those claims caused solely and completely by the negligence of ITAC Safety its directors, officers, employees or sub-contractors
