8-D is a quality management tool and a vehicle for a cross-functional team to articulate their thoughts and provide scientific solutions to
problems considering all the details. Organizations can benefit from the 8-D approach by applying it to all areas. The 8-D provides excellent
guides to get to the root cause of a problem and ways to verify that the solution actually works. Instead of fixing a symptom, the disease is
cured, therefore it is unlikely that the same problem will recur.

Planning Phase: Is an Emergency Response Plan necessary?
The problem-solving method is appropriate in “unknown
cause” situations and is not the appropriate tool if the
concern is focused solely on decision-making or problem
0 prevention. The 8-D is especially useful because it is not
only a problem-solving process but also a standard
reporting format. Does this problem require 8-D? If the
answer is yes, comment why and proceed.

Develop Containment Actions
Temporary actions to contain and “fix” the problem until the permanent solution is implemented and the actions have been documented.

5 Identify Permanent Corrective Actions
Solutions that correct the root cause. Solutions determined to be the best among all alternatives. Document and verify the Permanent
Corrective Action (PCA).
Prevent Recurrence
Determine which improvements in systems and processes will prevent the recurrence of the problem. Ensure that corrective actions remain
successfully implemented.
