Requirements for Admission into the ISPSC COLLEGE ADMISSION TEST 2019
1. Duly filled up application form (ISPSC CAT Form 1a)
2. 2 pieces identical photograph, passport size with name tag , taken within the last six months
3. Certification duly signed by the principal that the applicant is currently enrolled in the
high school and a member of the graduating class
4. If belonging to Top 10, certification duly signed by the principal that the applicant
belongs to the top 10 of the graduating class
5. Photocopy of Birth Certificate issued by NSO or PSA.
6. Photocopy of Form 138 for Senior High School graduates
7. Transfer Certificate/Transfer Credentials for Transferees.

HOW TO APPLY
1. Fill up application form completely and accurately and attach all requirements
2. Submit application at the Office of the Registrar or Head of the Testing Center at the campus
of your choice
